Clinical outcomes of combined presbyopia and astigmatism correction following hydrophilic trifocal toric intraocular lens implantation in cataract patients: a prospective clinical study

Abstract

Presbyopia and corneal astigmatism frequently coexist in cataract patients, reducing visual quality and increasing spectacle dependence. Toric trifocal intraocular lenses (IOLs) aim to address both conditions simultaneously. This prospective study evaluated the visual performance, quality, patient satisfaction, and safety profile of the Liberty 677MTY toric IOL, and classified its functional profile based on distance-corrected monocular defocus curve analysis. This prospective, single-center, non-comparative clinical investigation included 28 eligible cataract patients with corneal astigmatism (1.00–6.00 D) who underwent bilateral implantation of the Liberty 677MTY trifocal toric IOL. Uncorrected and distance-corrected visual acuities at all distances, subjective refraction, IOL rotational stability, defocus curves, contrast sensitivity, patient-reported visual function, and safety were reported during a 12-month follow-up period. The Liberty 677MTY IOL demonstrated significant improvements (p < 0.05) in uncorrected and distance-corrected visual acuities and subjective refractive cylinder, with stability over time. IOL alignment was stable through follow-up, with mean signed and absolute rotations of < 2° and < 5°, respectively. Binocular visual acuity defocus curves showed visual acuities better than 0.2 logMAR across the range of + 1.00 to ‑3.50 D. Monocular visual acuity defocus analysis confirmed full range of field smooth (FRoF-Sm) classification of the IOL. Contrast sensitivity defocus curve met acceptance criteria. Based on patient-reported outcomes 89% rated satisfaction as high, and 83% reported mild or no difficulty with visual disturbances. At 12 months, spectacle independence was achieved in 96.3% of patients for far, 81.5% for intermediate, and 92.6% for near vision. Safety analysis revealed one Nd: YAG-treated PCO case (1.61%) and no other IOL-related adverse events. This study provides a detailed evaluation of the Liberty 677MTY trifocal toric IOL, integrating real-life binocular visual experience with comprehensive monocular assessments. The lens demonstrated predictable refractive outcomes, excellent rotational stability, and consistently high-quality vision across all distances, with strong patient satisfaction. Classified as FRoF-Sm by monocular defocus curve analysis, the lens offers continuous functional vision. By integrating monocular and binocular perspectives, these findings offer a clearer understanding of the lens’s performance, supporting its use as a safe and effective solution for achieving spectacle independence and enhancing postoperative quality of life.

Clinical Performance of an Isofocal Toric Intraocular Lens with Double C-Loop Haptics: A Multicenter Retrospective Study

Purpose To evaluate clinical outcomes in eyes with pre-existing astigmatism undergoing cataract surgery or refractive lens exchange with the implantation of an isofocal optic-design toric intraocular lens (IOL) with double C-loop haptics. Method This multicenter retrospective study comprised 100 eyes (65 patients) implanted with the Isopure Serenity toric IOL (BVI, Inc.) conducted across four sites. Postoperative evaluation at between 2 and 3 months included manifest refraction, monocular uncorrected and distance corrected visual acuity (UDVA and CDVA), monocular uncorrected and distance corrected intermediate visual acuity (UIVA and DCIVA, at 80 and 66 cm), monocular uncorrected and distance corrected near visual acuity (UNVA and DCNVA, at 40 cm), binocular defocus curve and rotational stability. Results Manifest refraction spherical equivalent and refractive astigmatism were −0.24±0.41D and −0.36±0.36D, respectively. About 79% and 97% of eyes were within ±0.50D and ±1.00D, respectively, and 80% and 96% of eyes were ≤0.50D and ≤1.00D of refractive cylinder, respectively. At distance vision, the mean monocular CDVA was −0.02±0.06 logMAR and 92.93% of eyes showed a CDVA of ≥20/20. At intermediate vision, the mean monocular DCIVA values were 0.10±0.07 and 0.17±0.10 logMAR at 80 cm and 66 cm, respectively; and 96.92% and 88.33% of eyes showed a DCIVA of ≥20/32 at 80 cm and 66 cm, respectively. At near vision, the mean monocular DCNVA was 0.31±0.08 logMAR and 75.44% of eyes showed a DCNVA of ≥20/40. The binocular depth of focus range was approximately 1.50D. The mean absolute IOL rotation was 2.73±2.58 degrees with all eyes having a rotation of <10 degrees. Conclusions Eyes with pre-existing astigmatism implanted with the Isopure Serenity toric IOL showed good distance visual acuity and functional intermediate vision, with high refractive accuracy and good rotational stability following cataract and refractive lens exchange surgery.

Comparative study on rotational stability of toric intraocular lenses in cataract patients with low astigmatism: vertical versus horizontal implantation.

OBJECTIVE To compare the effects of horizontal versus vertical in-the-bag implantation of toric intraocular lenses (IOLs) on postoperative rotational stability, refractive outcome, and vision-related quality of life in cataract patients with low regular astigmatism, and to identify risk factors for postoperative rotational instability. METHODS This retrospective study included 370 eyes of 370 patients with low regular astigmatism who underwent phacoemulsification with toric IOL implantation. Patients were divided into horizontal implantation (n = 189) and vertical implantation (n = 181) groups according to intraoperative IOL orientation. Follow-up assessments were conducted at 1 week, 1 month, and 3 months postoperatively. The primary outcome was rotational instability at 3 months, defined as a rotation angle ≥ 5°. Secondary outcomes included rotation angle over time, uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), spherical equivalent (SE), residual astigmatism, and National Eye Institute Visual Function Questionnaire-25 (NEI VFQ-25) scores. Longitudinal changes were analyzed using a linear mixed-effects model (LMM), and logistic regression was used to identify risk factors. RESULTS Baseline and intraoperative data were comparable between groups. The LMM revealed significant effects of time, group, and their interaction on rotation angle (all P < 0.001). Rotation angles were significantly lower in the vertical implantation group than in the horizontal implantation group at all follow-up time points (all P < 0.001). At 3 months, rotational instability was lower in the vertical group than in the horizontal group (5.5% vs. 16.9%, P < 0.001). The vertical group demonstrated better UCVA, BCVA, SE, and lower residual astigmatism (all P ≤ 0.001), along with higher NEI VFQ-25 scores across multiple domains. Vertical implantation was independently protective (OR = 0.194, 95% CI: 0.077-0.444, P < 0.001), whereas longer axial length, shallower anterior chamber depth, and larger capsular bag diameter were identified as risk factors. Complication rates were similar between groups. CONCLUSION Vertical in-the-bag implantation of toric IOLs provided superior rotational stability, refractive outcomes, visual recovery, and vision-related quality of life compared to horizontal implantation in cataract patients with low regular astigmatism.

Clinical Outcomes after Bilateral Mix-and-Match Implantation of Extended Depth of Focus and Trifocal Intraocular Lenses
