Anterior horizontal rectus anatomy in children with concomitant strabismus.

Abstract

Background

Anterior ciliary vessel (ACV) counts, tendon width (TW), and limbus-to-insertion distance (LID) describe vascular, structural, and positional features of the anterior horizontal rectus. Data integrating these parameters in children with concomitant strabismus remain limited. We aimed to characterize ACV counts, TW, and LID in pediatric horizontal concomitant strabismus and examine their associations with diagnosis and clinical variables.

Methods

In this hospital-based cross-sectional study, 227 children undergoing primary horizontal rectus surgery were included. ACV bundles were quantified from intraoperative images, and TW and LID were measured. Esotropia-exotropia comparisons were performed within measured muscle subsets. Exploratory analyses examined associations with deviation magnitude, age, and axial length (AL), with false discovery rate (FDR) correction. Multivariable linear regression adjusted comparisons for age and AL. Logistic regression was performed in cases with complete measurements of both horizontal rectus muscles.

Results

The mean age was 7.31 ± 3.18 years. ACV-count patterns differed between the medial rectus (MR) and lateral rectus (LR), but not by diagnosis or deviation magnitude; 59.18% of LRs had two ACV bundles. Mean TW was 9.98 ± 0.65 mm for the MR and 9.60 ± 0.64 mm for the LR. Mean LID was 5.44 ± 0.38 mm for the MR and 6.84 ± 0.40 mm for the LR. After adjustment for age and AL, the crude LR width difference between exotropia and esotropia was attenuated (P = 0.212), whereas MR insertion remained shorter in esotropia (P = 0.022). LR width showed positive associations with age (P < 0.001) and AL (P = 0.004). Narrower MR width showed nominal associations with larger exodeviations in large-angle exotropia and did not remain significant after FDR correction. In restricted logistic regression, more anterior MR insertion was associated with esotropia (P = 0.033).

Conclusions

In pediatric concomitant horizontal strabismus, LR ACV bundles were more frequent than traditionally described, LR width appeared influenced by age and AL, and MR LID was shorter in esotropia than in exotropia. These integrated anatomical findings provide a useful surgical anatomical reference for children and may help frame future studies of esotropia-exotropia mechanisms and postoperative response.

Quantitative preoperative craniovertebral junction anatomy in Chiari I malformation: a comparative MRI study.

BACKGROUND Chiari I malformation (CM-I) is characterized by caudal displacement of the cerebellar tonsils through the foramen magnum. Craniovertebral junction (CVJ) anatomical variations may contribute to disease pathophysiology. This study is the first of a six-article institutional series investigating the anatomical substrate, surgical outcomes, and complications of CM-I. OBJECTIVE To quantitatively compare CVJ measurements between CM-I patients and healthy controls, and establish the morphometric baseline of a consecutive surgical cohort. METHODS A retrospective MRI-based study included 20 CM-I patients from a consecutive cohort of 42 operated patients (2012-2019) and 20 age- and sex-matched controls. McRae line length, bulbo-cervical angle, and tonsillar descent were measured on mid-sagittal T2-weighted MRI by two independent blinded observers. Inter-observer reliability was assessed by intraclass correlation coefficient (ICC); significance was set at p <0.05. RESULTS McRae line length was significantly greater in CM-I patients (34.41 ± 2.29 mm) than controls (28.98 ± 2.80 mm; p <0.0001; 95% CI: 4.13-6.73 mm). Mean tonsillar descent was 7.52 ± 0.98 mm (range: 5.3-8.2). ICC was 0.94 (95% CI: 0.89-0.97). No significant between-group difference was observed in bulbo-cervical angle. The cohort comprised 55.0% females (mean age: 32.5 years); occipital and Valsalva-induced headache were present in 78.6% and 81.0%, respectively. CONCLUSION CM-I is associated with significant foramen magnum enlargement, supporting the role of posterior cranial base morphology. McRae line enlargement was confirmed as an independent predictor of surgical complications (OR 4.2; p=0.037).

Combined Orbital Decompression and Rectus Muscle Myectomy for Large-Angle Strabismus in Thyroid-Associated Orbitopathy.

INTRODUCTION Thyroid-associated orbitopathy (TAO) often causes restrictive strabismus and diplopia. Large-angle deviations pose particular challenges owing to extraocular muscle fibrosis and unpredictable surgical dose-response. Although combined orbital decompression and strabismus correction has been proposed to reduce the number of procedures, its effectiveness in large-angle TAO remains uncertain. This study aimed to evaluate the outcomes of combined orbital decompression and rectus muscle myectomy in this population. METHODS In this retrospective cohort study, 88 patients with TAO-associated strabismus > 25 prism diopters (PD) underwent either staged (step) surgery (n = 42) or combined surgery (n = 46) at a tertiary center between 2018 and 2022. All had stable thyroid function for ≥ 6 months before surgery. The combined procedure involved simultaneous orbital decompression and rectus myectomy. Outcomes included deviation angles, Hertel exophthalmometry, and the field of binocular single vision (BSV). RESULTS Baseline characteristics were comparable between groups. Postoperatively, the combined-surgery group had smaller residual horizontal and vertical deviations-median 0 (0-3) and 2 (0-3) PD-than the step group, 5 (4-5) and 3 (3-5) PD (all p < 0.001). The BSV field was wider after combined surgery [410 (396-426)° versus 360 (328-386)°, p < 0.001]. Overall success rates (≤ 5 PD deviation in primary and reading positions) were similar (84.8% versus 81.0%, p = 0.633). Transient lower lid retraction occurred in a few patients and was successfully corrected. CONCLUSIONS Combined orbital decompression with rectus myectomy achieved effective alignment and superior binocular function compared with staged surgery, offering a practical and safe option for TAO-related large-angle strabismus.
