Assessment of the Reliability and Diagnostic Accuracy of Indirect MRI Signs for ACL Tears: Comparison with Arthroscopic Findings as the Gold Standard

Abstract

Background: The objective of this study was to evaluate the sensitivity and specificity of indirect MRI signs (PCL Angle, PCL Inclination Angle, PCL-PCA, and Anterior Drawer Sign) in diagnosing ACL tears, as confirmed by arthroscopy. Methodology: This retrospective case-control study, conducted at Shifa International Hospital in Islamabad from January to December 2023, included 100 patients divided into two groups based on arthroscopy findings: 50 with a healthy ACL (Control) and 50 with ACL deficiency (Test). MRI signs were analyzed retrospectively to calculate cut-off values, sensitivity, specificity, and AUC. Results: The study involved 100 patients (77 males, 23 females), with a mean age of 35.12 years (range 16–75). Among MRI parameters, PCL-PCA was the most sensitive (81.3%, AUC 0.821), followed by PCL angle (78.1%, AUC 0.870) and PCL inclination angle (68.8%, AUC 0.697). The Anterior Drawer Sign showed high specificity (89.19%) but lower sensitivity (57.14%) and was statistically significant (p = 0.001). Conclusion: The crux of this study is that PCL-PCA is highly sensitive in diagnosing ACL tear as an indirect and secondary parameter, followed by PCL angle, then by PCL inclination angle, and lastly by anterior drawer sign. These provided an excellent distinction between patients with and without an ACL tear.

A Prospective Study to Evaluate the Efficacy of Clinical Assessment, MRI, And Arthroscopic Findings in Meniscal Tear

Background: These tears of the meniscus are one of the most common intra-articular knee conditions, causing pain, dysfunction and disability. Proper diagnosis is crucial to proper management. The purpose of this study was to prospectively assess and make comparisons between the diagnostic value of the clinical examination, MRI and arthroscopy evaluation in the patient presenting with a suspected meniscal tear. Material and Methods: A prospective study was carried out in patients with clinical presentation suggestive of meniscal injury. After detailed clinical examination and MRI, the patients all had arthroscopy. The sensitivity, specificity, and overall diagnostic (accuracy) accuracy of clinical evaluation and MRI was determined compared to arthroscopic diagnosis. The reference standard was considered to be the findings. Results: Clinical examination proved to be very sensitive but not quite as specific as MRI. MRI was more accurate than other methods in the diagnosis of medial and lateral tears. Arthroscopy confirmed the pitfalls of clinical and radiographic examinations, and emphasized the need for arthroscopy as the ultimate diagnostic and therapeutic tool. Conclusion: Clinical assessment and MRI are useful, non-invasive means of evaluating meniscal tears with MRI having greater overall diagnostic accuracy. But definitive diagnosis and management cannot be achieved without arthroscopy. Keywords: Meniscal tear, Meniscal injury, Arthroscopy (Gold Standard).

Mind the Notch: MRI Clues to Forecasting Unilateral and Bilateral ACL Injuries Using Notch Width Index

A BSTRACT Anterior Cruciate Ligament (ACL) injuries are prevalent in the physically active population. Anatomical risk factors such as the Notch Width Index (NWI), the ratio of intercondylar notch width to bicondylar width, have been associated with ACL injury susceptibility. However, limited evidence exists on the predictive role of NWI in bilateral ACL injuries, which may reflect a distinct high-risk profile. A retrospective cross-sectional study was conducted on 141 individuals who underwent knee MRI. 70 had no ACL injuries (controls), while 71 had complete ACL tears. NWI was measured on MRI at the level of the popliteal groove. Two blinded radiologists calculated NWI. Multinomial logistic regression assessed the association between NWI and ACL injury type, using controls as reference. The mean age was 33.96 ± 12.12 years, with 83 males. Mean NWI was 0.259 ± 0.037. Lower NWI values were significantly associated with unilateral ( P = 0.001; OR = 1.342 × 10²²) and bilateral ACL injuries ( P = 0.001; OR = 3.384 × 10 32 ). The model showed good fit ( P = 0.985) with moderate explanatory power (Nagelkerke R ² =0.321). Stem-and-leaf plots demonstrated a progressive narrowing of NWI across groups. MRI-based NWI is a strong predictor of both unilateral and bilateral ACL injuries. Bilateral ACL injury cases may represent a distinct high-risk anatomical phenotype. NWI could serve as a valuable screening tool to identify at-risk individuals for targeted preventive interventions.
