Invasive pulmonary aspergillosis can serve as a key sentinel manifestation leading to the diagnosis of Good syndrome in patients with thymoma in patients with thymoma.
Abstract
Background Good syndrome is a rare adult-onset combined immunodeficiency associated with thymoma, characterized by hypogammaglobulinemia and impaired T-cell–mediated immunity. Opportunistic infections are a major cause of morbidity and mortality, and invasive aspergillosis may represent a severe but underrecognized manifestation. In some cases, it may serve as a sentinel presentation leading to the diagnosis of Good syndrome. Case presentation A 52-year-old man presented with a history of thymoma and myasthenia gravis, chronic sinopulmonary infections, cytomegalovirus colitis, chronic diarrhea, and cytopenia. Chest CT scan revealed multiple pulmonary nodules and tree-in-bud opacities. Bronchoalveolar lavage, galactomannan assay, and lung biopsy confirmed Aspergillus infection. Subsequent comprehensive immunologic evaluation demonstrated hypogammaglobulinemia, markedly reduced peripheral B cells (CD19⁺), and impaired T-cell immunity, consistent with Good syndrome. Conclusion Invasive pulmonary aspergillosis can serve as a key sentinel manifestation leading to the diagnosis of Good syndrome in patients with thymoma. Early recognition of opportunistic infections and prompt immunological assessment are critical for timely treatment and improved clinical outcomes.
The case of a previously healthy 32‐year‐old man who presented with a mononucleosis‐like syndrome complicated by rapidly progressive hypoxemic respiratory failure requiring invasive mechanical ventilation is reported, highlighting the diagnostic challenges of pulmonary toxoplasmosis in immunocompetent hosts.

This case underscores the lethal potential of concurrent Aspergillus and Burkholderia infections in X-linked CGD and highlights the critical importance of early diagnosis, which can be achieved through functional assays such as the DHR test or NBT test, followed by genetic confirmation when available.

Good’s syndrome (GS) is a rare immunodeficiency disorder associated with thymoma, characterized by hypogammaglobulinemia and impaired cellular immunity. Due to its variable clinical presentation and lack of clear diagnostic criteria, GS is often underrecognized or diagnosed with delay.

Invasive pulmonary aspergillosis (IPA) presents life-threatening complication mostly seen in patients with severe neutropenia or in patients with solid organ cancers after applied anticancer therapy. The aim of this manuscript is to identify clinical manifestations of IPA in lung cancer patient. A 65-year-old male with...

Rationale: Behçet’s disease (BD) is a systemic vasculitis that rarely involves the lungs. Pulmonary manifestations of BD can radiographically mimic infectious pneumonia, posing a substantial diagnostic challenge and increasing the risk of inappropriate treatment. Clinicians should consider mucormycosis in patients with poor response to voriconazole and progressive pulmonary lesions, and early bronchoscopy with mNGS is recommended, although isavuconazole is considered relatively safe.
