P-wave duration is concluded to be a sensitive, readily available, non-invasive biomarker of preclinical cardiac remodeling in Shih Tzu dogs, enhancing tools for early risk stratification.

Abstract

ABSTRACT Degenerative mitral valve disease (DMVD) is the most prevalent canine cardiac condition, with left atrial enlargement (LAE) being a critical predictor of progression to congestive heart failure (CHF) and arrhythmogenesis. Identifying accessible, non-invasive markers for preclinical according with the American College of Veterinary Internal Medicine - ACVIM (ACVIM Stage A) remodeling is paramount for timely risk stratification. This study investigated the relationship between electrocardiographic (ECG) P-wave duration and key echocardiographic metrics: indexed left atrial end-systolic volume (LAESV IDX), LA/Ao ratio, and normalized Left Ventricular internal diameter (LVIDDn), in a tightly controlled cohort of 22 Shih Tzu dogs (ACVIM Stage A). The mean P-wave duration was 39.55±3.63 ms. Significant positive correlations were found with all remodeling parameters (p<0.05), being the strongest with LAESV IDX (r = 0.61; p=0.002). Linear regression established LAESV IDX as the most robust predictor of P-wave duration (adjusted R²=0.342; p=0.002). Reference thresholds were derived based on ACVIM criteria and sample P95 (LAESV IDX: 44 ms; LA/Ao: 49ms). A transition zone (44-49 ms) suggestive of incipient LAE is proposed. P-wave duration is concluded to be a sensitive, readily available, non-invasive biomarker of preclinical cardiac remodeling in Shih Tzu dogs, enhancing tools for early risk stratification.

Echocardiographic Left Atrial Volumetric-mechanical Coupling Index for Prediction of Atrial Fibrillation after STEMI.

BACKGROUND Atrial fibrillation (AF) commonly complicates ST-segment elevation myocardial infarction (STEMI). Conventional echocardiographic measures may incompletely capture left atrial remodeling. The left atrial volumetric-mechanical coupling index (LACI-vm), integrating left atrial volume index (LAVI) and mitral ann...

Physiological Left Atrial Staging and the Risk of New-Onset Atrial Fibrillation in Hypertrophic Cardiomyopathy.

The proposed LA staging framework offers mechanistic insights into how the LA adapts to progressive diastolic dysfunction in HCM and is associated with a graded risk of NOAF and may help tailor AF surveillance strategies.

Left atrial dimension and new-onset atrial fibrillation in chronic Chagas disease: A competing-risk analysis of the SEARCH-Rio cohort.

In CCD, baseline LA diameter predicts new-onset AF over 25 years, with 4.0 cm acting as a dynamic threshold, and P-SAECG is unrelated to LA size and poorly predicts AF.

Left Atrial Strain as an Integrative Marker of Diastolic Dysfunction and Atrial Fibrillation in Hypertrophic Cardiomyopathy: A Retrospective Cross-Sectional Study

Background and Objectives: Echocardiographic assessment of left ventricular diastolic dysfunction (LVDD) in hypertrophic cardiomyopathy (HCM) is challenging, as conventional parameters correlate poorly with left ventricular filling pressure. Emerging evidence suggests that left atrial (LA) strain is associated with atr...

Graded MESA-LVH as a marker of left ventricular electrical remodeling: spectrum-based diagnosis, prognosis analysis.

BACKGROUND Emerging evidence suggests that electrocardiographic (ECG) voltage reflects left ventricular electrical remodeling (LVER) beyond left ventricular hypertrophy (LVH), with independent clinical and prognostic significance. OBJECTIVE To determine whether graded increases in QRS amplitude provide diagnostic and...
