Abstract
The acceleration of global health digitalization offers significant potential to improve public care quality; however, its practical execution frequently encounters multidimensional barriers and threatens to widen the digital health divide. Objective: This study aims to comprehensively evaluate the policy typologies, service modalities, contemporary challenges, and real-world implications of international digital health implementations. Method: Utilizing a Systematic Literature Review (SLR) design aligned with PRISMA 2020 guidelines, literature searches were performed across PubMed, ScienceDirect, and Google Scholar databases for articles published between 2022 and 2026. Applying rigorous selection via the PICOS framework, 8 core articles were retrieved for extraction using thematic analysis. Results: The synthesis reveals that macro-level policies successfully diversified digital services into administrative (e-prescription), remote clinical care (Remote Patient Monitoring), and preventive governance (Digital Proximity Tracing). Instrumentally, e-health effectively reduces operational costs, enhances clinical quality, and bridges access gaps for marginalized groups, such as coastal and migrant populations. Nevertheless, the transition is severely hindered by fragmented data interoperability, insufficient digital literacy among professionals, ethical issues concerning AI algorithmic biases, and policy blindness toward technological determinism, which drives social exclusion. Conclusion: Integrating social determinants into digital health architecture is vital for establishing an equitable public health ecosystem. Policymakers must devise equity-centered regulations and system standardization, while medical institutions are urged to adopt structured literacy training to minimize internal resistance.
Digital transformation can strengthen tuberculosis control through adherence monitoring, patient–health worker communication, recordkeeping, and service follow-up. However, its implementation in low- and middle-income countries (LMICs) continues to face policy challenges and structural barriers. This systematic review aimed to identify policy challenges, structural barriers, and strategies supporting digital transformation in tuberculosis control in LMICs. The review followed PRISMA 2020 and was registered with the International Prospective Register of Systematic Reviews (PROSPERO; CRD420261377489). We searched PubMed, Scopus, ScienceDirect, and Springer Nature Link for peer-reviewed articles published between 2015 and 2025. The database searches initially displayed 859 records, of which 809 were successfully exported and imported into Mendeley. After removing 19 initial duplicates, we screened 790 records using the SPIDER framework. We assessed methodological quality using the JBI Critical Appraisal Checklist for Qualitative Research. Nineteen studies met the inclusion criteria and were thematically analyzed. Policy challenges involved regulation and data governance, policy communication, coordination across sectors and levels of care, and digital program integration. Structural barriers included limitations in the health workforce, technological infrastructure, financing, digital access and equity, health information systems, and service delivery. Recommended strategies comprised stronger regulation, system integration, sustainable financing, workforce development, patient-centered approaches, and monitoring and evaluation. Digital transformation in tuberculosis control requires an integrated, secure, sustainable, equitable, and context-responsive health system. Stakeholders should prioritize data protection, interoperability, sustainable financing, continuous training, and non-digital service options to prevent technology from widening inequalities in access.
Background: Digital transformation has become a strategic priority for healthcare organizations, driven by the growing adoption of digital health technologies, telemedicine, artificial intelligence, and health information systems. Despite increasing scholarly attention, the literature remains fragmented across disciplines, limiting a comprehensive understanding of its intellectual structure, dominant themes, and future directions.Aims and Scope of the Paper: Therefore, this study systematically examines the development of digital transformation research in healthcare management using a combined bibliometric and systematic literature review approach.Methods: Data were retrieved from the Scopus database and screened using the PRISMA framework, resulting in 62 publications published between 2016 and 2026. Bibliometric analysis was conducted using VOSviewer to explore publication trends, collaboration networks, citation structures, and thematic relationships, while thematic synthesis was used to identify major research themes and emerging topics.Result: The findings indicate substantial growth in research on digital transformation in healthcare management, particularly after 2022, accompanied by increasing international collaboration and citation activity. Healthcare management, digital health, and telemedicine emerged as the most prominent themes. Keyword co-occurrence analysis identified five major research clusters: Digital Health Transformation, Telemedicine and Remote Healthcare Services, Health Information Systems and Data Governance, Artificial Intelligence and Healthcare Innovation, and Workforce Development, Leadership, and Organizational Readiness. Overlay visualization further highlighted emerging interests in workforce competency, digital leadership, health information management, and data security.Conclusion: This study provides a comprehensive mapping of the intellectual structure of digital transformation research in healthcare management and identifies key research gaps related to workforce readiness, artificial intelligence governance, healthcare cyber security, and sustainable digital transformation strategies. The findings offer valuable insights for researchers, healthcare managers, and policymakers seeking to advance healthcare digitalization initiatives.

The healthcare sector contributes significantly to environmental degradation, necessitating the integration of Green Human Resource Management, yet its implementation in regional hospitals still faces significant gaps. This study aimed to explore the perceptions and realities of GHRM implementation in the healthcare sector. A mixed-methods research approach integrating Systematic Literature Review, bibliometrics, and qualitative studies was conducted at dr LM. Baharuddin M.Kes Regional General Hospital. involving four informants through purposive and snowball sampling. Data collection was conducted through in-depth interviews, observation, and documentation, analysed using thematic analysis with the help of NVivo 15, complemented by a bibliometric of 22 articles using VOSviewer. The results show that GHRM experienced significant acceleration across disciplines in 2024-2026, but implementation in the field is still incidental, based on appeals, and has not been integrated into formal HR instruments such as SKP, reward systems, or promotions. Hospital management needs to systematise environmental metrics into formal performance evaluations. Future research uses a longitudinal quantitative approach with SEM to test psychological mediating variables. In conclusion, this study extends the AMO framework to the context of hospitals in developing countries. Recommendation: Structural integration of GHRM into formal HR systems is necessary to achieve sustainable healthcare.

Introduction Health digitalization entails a complex array of actors and roles, including healthcare stakeholders and commercial digital-health innovators. The implementation of digital health innovations (DHIs) is notoriously prone to failure. Consequently, many theories, models, and frameworks (TMFs) have been developed to explain or guide the process of health digitalization. Depending on their scholarly origins, these TMFs differ in their perspective, origin, and terminology, leading to knowledge fragmentation and confusion. The aim of this review was to create an overview of the available TMFs, bringing together and clarifying different perspectives of healthcare stakeholders and innovators during the DHI life cycle. Methodology This study focused on TMFs describing, explaining, predicting, or prescribing parts of the innovation pathway considering influential factors and actors. Three databases (PubMed, EMBASE, and Web of Science) were searched using the aid of an information specialist in February 2025. We identified relevant keywords for the search by extensively reviewing eHealth and health digitalization literature. Studies were eligible for inclusion if they were published after 2000, written in English, and presented a new or significantly adapted TMF empirically tested in healthcare settings involving a commercial DHI. Studies applying only a known TMF without contextual adaptation or a DHI in the prototype phase were excluded. Results From the 4628 potentially relevant articles, 12 fulfilled the inclusion criteria. The included TMFs came from diverse scholarly disciplines, pointing to the multidisciplinary knowledge needed for health digitalization. Four TMFs focused on behavior change of end users, two TMFs explored organizational adoption of the DHI, and six TMFs dealt with factors that shape innovation design and development. Several constructs were found in two or all of the TMF groups. The actors involved came from diverse backgrounds and differed according to the type of DHI and scope of implementation. Conclusion The included TMFs synthesized foundational theories and constructs from specific problem areas in health digitalization. Analyzing the reasons for adding certain constructs indicated how actors and researchers in the different groups perceive the landscape of health digitalization and the realities that shape the digitalization process, helping to create a holistic view aiding continuity and outward growth of the digitalization team. Lean healthcare has emerged as a transformative approach to enhance efficiency, reduce waste, and improve patient outcomes in hospital settings. This systematic review aims to identify the key issues and challenges in implementing Lean initiatives in hospitals. Guided by the PRISMA framework, an extensive literature search was conducted across PubMed, Scopus, and Web of Science for literature published between January 2015 and December 2023, ultimately including 15 studies. The review categorises challenges into seven primary themes: resource limitations, organisational resistance, knowledge and training gaps, operational inefficiencies, communication barriers, sustainability concerns, and patient-centric obstacles. Study quality and risk of bias were independently assessed using the Joanna Briggs Institute (JBI) and the Center for Evidence-Based Management (CEBMa) critical appraisal tools. Resource constraints, encompassing financial, infrastructural, and human resource shortages, emerged as the most prominent barrier, reported in 66.7% of studies. Organisational resistance, driven by scepticism, fear of job losses, and insufficient leadership support, was the second most frequent challenge, found in 46.7% of studies. Additionally, gaps in staff knowledge and training regarding Lean principles, alongside workflow inefficiencies, further hindered implementation. Communication and coordination issues, often exacerbated by hierarchical decision-making, and challenges sustaining Lean practices due to high staff turnover and shifting priorities were also highlighted. Balancing Lean’s efficiency goals with the need for personalised patient care posed additional complexities. The findings underscore the necessity for comprehensive strategies to address these barriers. Strong leadership commitment, tailored training programs, and resource optimisation are critical for successful and sustainable Lean implementation. While Lean offers significant potential to enhance hospital operations and patient care, overcoming these multifaceted challenges is essential for realising its full benefits. This review provides actionable insights for policymakers and healthcare administrators, aiming to foster effective, context-sensitive strategies to mitigate barriers and promote Lean healthcare adoption.

The Non-communicable diseases (NCDs) account for 71% of global deaths and require digital transformation in management at the primary healthcare level. Analyze the impact of digital health information systems on the effectiveness of patient-centered health promotion strategies for NCDs in public health clinics. The Systematic Literature Review uses the PRISMA 2020 methodology with searches on 7 electronic databases (Scopus, ScienceDirect, Frontiers, SpringerLink, Heliyon, Taylor & Francis, PubMed) for the period 2020-2025. Of the 309 articles identified, 40 studies met the inclusion criteria. Digital platforms integrated with robust eHealth infrastructure indicate significant effectiveness in improving patient health outcomes, with hypertension control reaching 59% and mental health improvement up to 86%. Patient-centered communication and eHealth are key moderators in increasing patient engagement. The implementation of digital health information systems has a positive impact on patient-centered health promotion for NCDs. A systemic approach that integrates technology, human resources, and comprehensive policies is needed to optimize digital health transformation at the community level.
