TL;DR

It is concluded that paravertebral block provides superior pain relief, improves respiratory mechanics, accelerates early mobility, and reduces complications and hospital stay in patients with multiple rib fractures.

Abstract

Objective: Multiple rib fractures are associated with significant pain, impaired respiratory function, and increased risk of complications. To assess the impact of the thoracic paravertebral block on pain control, respiratory function, early ambulation, opioid consumption, and complication rates in patients with multiple unilateral rib fractures. Methods: This prospective, randomized controlled study was conducted at a tertiary trauma center. Eighty adult patients with ≥3 unilateral rib fractures were randomly assigned to two groups: Group A received thoracic PVB with local infiltration into the respective ICS, and Group B received standard systemic analgesia using intravenous opioids and NSAIDs. Results: Patients in Group A experienced significantly lower pain scores at all-time points (mean VAS at 72h: 2.1 vs 4.2, p<0.001). Incentive spirometry volumes improved more in Group A (1120 mL vs 880 mL, p<0.001). Time to ambulation was shorter (28.6 vs 42.4 hours, p<0.001), and opioid consumption was nearly halved (18.3 mg vs 36.9 mg morphine equivalent, p<0.001). Pulmonary complications were less frequent in Group A (atelectasis: 7.5% vs 25%, p=0.03). ICU and hospital stays were significantly shorter, and patient satisfaction scores were higher in the PVB group (p<0.001). Conclusion: It is concluded that paravertebral block provides superior pain relief, improves respiratory mechanics, accelerates early mobility, and reduces complications and hospital stay in patients with multiple rib fractures. Incorporating PVB into trauma care pathways offers a safe, effective, and patient-centered approach to rib fracture management.

Effect of derotation boot versus skin traction on pain in patients with intertrochanteric fractures a randomized controlled trial

Intertrochanteric hip fractures in older adults are often associated with severe pain, and inadequate preoperative analgesia increases the risk of delirium, immobility, and mortality. Although skin traction (ST) is widely used for temporary stabilization, its analgesic effectiveness remains poorly supported, and it m...

Thoracic Paravertebral Block Versus Erector Spinae Plane Block for Postoperative Analgesia in Video-Assisted Thoracoscopic Surgery: A Retrospective Cohort Study

Objectives: Thoracic paravertebral block (TPVB) and erector spinae plane block (ESPB) are widely utilized regional analgesia techniques for video-assisted thoracoscopic surgery (VATS). However, evidence comparing their analgesic efficacy remains inconclusive. This study aimed to compare postoperative opioid requirement...

Feasibility and Impact of Out-of-Theatre Regional Anesthesia for Rib Fractures: A Retrospective Cohort Study

Introduction Rib fractures cause significant morbidity due to pain-induced respiratory issues. While regional anesthesia (RA) provides excellent pain relief, evidence for its administration outside of theatres remains limited. This study aimed to evaluate the feasibility and clinical impact of out-of-theatre RA for tra...

Lumbosacral erector spinae plane block for hip fractures in the elderly: a two-center case series and descriptive analysis

Hip fractures in frail elderly patients pose significant anesthetic challenges due to comorbidities and contraindications to neuraxial techniques. This two-center retrospective case series evaluated the feasibility of combined lumbosacral erector spinae plane (ESP) block with femoral or pericapsular nerve group (PENG)...

Rib fixation still helps: Surgical stabilization of rib fractures in traumatic brain injury, a TQIP Study.

BACKGROUND Surgical stabilization of rib fractures (SSRF) has been associated with lower odds of mortality and reduced pulmonary morbidity in patients with chest trauma, but its role in patients with concomitant traumatic brain injury (TBI) remains controversial. We hypothesized that TBI phenotype modifies the associat...

Balloon kyphoplasty for traumatic thoracolumbar wedge compression fractures without neurological deficit: a case series of 12 patients

Background: Balloon kyphoplasty is well established for osteoporotic vertebral compression fractures. Its use in traumatic, non-osteoporotic thoracolumbar wedge compression fractures without neurological deficit is less understood, especially across a wide age range. Case series: We reviewed 12 patients (9 male, 3 fema...
