Rationalising the ERCP pathway and implementing an urgent cholecystectomy service would result in significant cost savings which could support the development of business cases.

Abstract

Due to insufficient resources, our institution largely employs a non-operative approach to acute gallstone presentations. The aim of this service review was to establish the impact of this on the endoscopic retrograde cholangiopancreatography (ERCP) service and associated bed days. 13 codes were used to identify symptomatic gallstone patients admitted to general surgery in March 2024. The first presentation of the patient's gallstones was identified, the timeframe of this was 26/04/2017 – 09/05/2024. ERCP data were extracted for all patients requiring an ERCP on the first admission or subsequent admissions. Data was analysed in Excel. 250 records were identified. Following exclusion, 72 patients were identified. In subsequent readmissions 82.6% (19/23) were performed as outpatient. The median time to ERCP from request was 10 days in both cohorts (Range 2-14 days in first admission and 2-29 days in readmission.) Wait time for ERCP accounts for a substantial number of inpatient bed days and the impact of not operating acutely increases the burden on ERCP services. Rationalising the ERCP pathway and implementing an urgent cholecystectomy service would result in significant cost savings which could support the development of business cases.

Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital ti...

Should We Be Aiming For A "Stent-first Strategy" in the Management of Malignant Biliary Obstruction? A Retrospective Single-Center Analysis.

BackgroundPercutaneous transhepatic cholangiography (PTC) is essential when endoscopic retrograde cholangiopancreatography (ERCP) fails or is contraindicated. Despite emerging evidence favoring primary stenting, data directly comparing it with the traditional two-staged drainage approach remain scarce.PurposeTo evaluat...

Recurrent Small Bowel Obstruction Following Contrast Challenge Versus Surgical Intervention at Index Presentation:1-Year Follow-Up of the Adhesive Bowel Obstruction in Children (ABC) Study.

AIM Long-term outcomes after contrast challenge (CC) in pediatric adhesive small bowel obstruction (ASBO) are unknown. This study assesses ASBO recurrence and need for surgical intervention within 1-year of ASBO admission. METHODS We performed a multi-institutional prospective implementation study of a standardized C...
