TL;DR

Suicidal ideation and depressive severity are key transdiagnostic predictors of suicide attempts in early-onset mood disorders, while alcohol use, impulsivity, psychiatric family history, and illness burden mark diagnosis-specific risk profiles guiding tailored monitoring and interventions.

Abstract

Objective To identify diagnosis-specific clinical and longitudinal predictors of suicide attempts in early-onset mood disorders, comparing major depressive disorder (MDD), bipolar I disorder (BD-I), and bipolar II disorder (BD-II) within a single cohort to inform targeted risk stratification. Methods We analyzed 430 patients aged under 35 years (a proxy for early-onset) with MDD, BD-I, or BD-II from a prospective South Korean cohort. Diagnoses were based on Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) and the Mini International Neuropsychiatric Interview. Lifetime and follow-up suicide attempts and suicidal ideation (Beck Scale for Suicide Ideation, SSI-beck) were assessed using structured interviews and standardized scales. Logistic regression examined predictors of lifetime suicide attempts, and Cox proportional hazards models evaluated baseline predictors of time to first suicide attempt over approximately 60 weeks of follow-up. Results At baseline, 40.5% of patients reported a lifetime suicide attempt, and 62 patients attempted suicide during follow-up. In the total patients, earlier age at onset, longer untreated duration, alcohol use problems, psychiatric family history, and higher SSI-beck scores were associated with lifetime suicide attempts. Diagnosis-specific analyses showed distinct patterns. Prospectively, depressive severity and SSI-beck scores predicted first suicide attempts, with alcohol use and impulsivity additionally important in MDD. Conclusion Suicidal ideation and depressive severity are key transdiagnostic predictors of suicide attempts in early-onset mood disorders, while alcohol use, impulsivity, psychiatric family history, and illness burden mark diagnosis-specific risk profiles guiding tailored monitoring and interventions.

Depression and risk of future suicidal behaviour in patients with deliberate self-harm.

BACKGROUND Depression is strongly associated with suicidal behaviour, but whether major depressive episode (MDE) diagnosis and depressive symptom severity provide equivalent prognostic information after self-harm remains unclear. METHODS 739 adults were interviewed at three Swedish hospitals after an episode of delib...

Suicidal ideation at psychiatric emergency presentation predicts trajectories of functional disability and symptom burden in mood and anxiety disorders: A prospective cohort study.

Suicidal ideation (SI) is common among patients presenting to psychiatric emergency departments (EDs), yet its association with longer-term clinical and functional outcomes remains insufficiently understood. This study examined trajectories of functional disability, depressive and anxiety symptoms, and alcohol misuse a...

Prospective associations of depression and anxiety symptoms with the transition from suicidal ideation to attempt over 18 months.

BACKGROUND Anxiety and depressive disorders are known risk factors for suicide. However, limited prospective research has tested whether mental disorders are associated with transitions from suicidal ideation (SI) to suicide attempt (SA). Our primary aim was to test the effects of mental disorder symptoms on SA over 18...

Recurrence of suicidal episodes in individuals with probable borderline personality disorder: one-year follow-up study

Background Borderline personality disorder (BPD) is strongly associated with suicidal behavior and high rates of reattempts. Short-term prediction of recurrent emergency presentations after a suicidal crisis in individuals with BPD traits remains clinically challenging, despite extensive study of suicidality in BPD mor...

Predicting suicide attempts in people with autism and co-morbid mood or psychotic disorders

Individuals with autism spectrum disorder (ASD) and co-occurring mood or psychotic disorders face substantially elevated suicide-attempt risk, but prior research is largely limited to population-level incidence and cross-sectional risk-factor studies, leaving unclear whether individual-level, prospective prediction is...

Anxiety disorders, PTSD, and risk of first-ever stroke: A meta-analysis of 11 million participants.

BACKGROUND Anxiety, obsessive-compulsive disorder (OCD), and post-traumatic stress disorder (PTSD) involve chronic fear and stress-related dysregulation that may increase vascular risk, yet their association with stroke and stroke type has not been systematically evaluated. We aimed to quantify associations between anx...
