The Association Between Knee Osteoarthritis Risk Factors and Quality of Life Among Patients with Knee Osteoarthritis in Ubung Village

Abstract

Osteoarthritis is a degenerative joint disease that affects the synovial membrane, cartilage, ligaments, and bones. Several risk factors for osteoarthritis include increasing age, family history of generalized osteoarthritis, heavy physical activity, obesity, and previous joint trauma or deformity. This study aimed to identify the knee osteoarthritis risk factors associated with patients’ quality of life. A cross-sectional study was conducted among 91 patients with knee osteoarthritis in Ubung Village, Denpasar City. Data were collected using the Short Form-36 (SF-36) questionnaire to assess the quality of life of the patients. Body weight was measured using a weighing scale, and height was measured using a stadiometer to determine body mass index (BMI). The results showed that several factors were significantly associated with poor quality of life among patients with knee osteoarthritis in Ubung Village, including age >60 years (odds ratio [OR]=4.331), female sex (OR=5.571), excess body weight (OR=7.353), and vigorous physical activity (OR=3.368). Overall, 73.6% of patients demonstrated a good quality of life. However, domain-specific analysis revealed moderate to low median scores in several components, including work limitations due to physical health disorders, pain, and general health. Bivariate analysis furtherdemonstrated that age >60 years, female sex, overweight or obesity status (BMI >23 kg/m²), occupations involving heavy physical activity, and a history of knee trauma were significantly associated with poor quality of life.

Association Between Center of Pressure Parameters and Self-reported factors in Osteoarthritis Across Age and BMI: A cross-sectional Study in East Azerbaijan, Iran.

Knee osteoarthritis (KOA) is a common degenerative joint disease, especially in older adults, affecting postural stability and center of pressure (COP) behavior. These biomechanical parameters may vary by age, gender, and body mass index (BMI). This study aimed to explore the relationship between COP features and these individual factors, along with self-reported pain and function, in people with KOA. From a cohort of 4,355 individuals in East Azerbaijan, 333 self-reported a physician diagnosis of KOA. After excluding cases with comorbidities, 75 participants were included. Pain and function were assessed using the WOMAC and Visual Analog Scale (VAS), while COP metrics were collected using a PT-Scan® pressure platform. One-way ANOVA and Pearson correlation were used for analysis. Age demonstrated positive relationships with Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) subscales in men (Pain LSD=341.16, p<0.001; Total LSD=2333.69, p<0.001 and Functional LSD=1391.6, p<0.001). In women, age showed positive effects on WOMAC Total (LSD=60.00; p<0.05). Body mass index (BMI) showed negative effects on WOMAC Stiffness in both genders (Men LSD=125, p<0.001; Women LSD=150, p<0.001). In examining the relationship between the characteristics of the COP and age in women, a significant relationship was seen between age and a number of characteristics of the COP, including anterior-posterior range (AP-Range-R), mediolateral range (ML-Range-L), and the area of the confidence circle (AREA_CC_L). Examining the correlation between the characteristics of pain and the COP, a significant negative correlation was found between VAS and the characteristic of the COP (fractal dimension (FD_CE_L)). in women. These results highlight the need for age- and gender-specific approaches in evaluating balance and function in KOA. .

Magnitude of weight loss and structural progression of knee osteoarthritis in adults with obesity - A prospective cohort study

Background: Obesity is the major modifiable risk factor for knee osteoarthritis (OA) but the amount of weight loss needed to reduce structural progression in the real world is not well defined. Materials and Methods: In this single-center prospective cohort study, 400 adults with obesity (body-mass index [BMI] (cid:21) 30 kg/m 2 ) and Kellgren–Lawrence grade 2–3 knee OA were enrolled and assessed at baseline and 12 months. The main outcome was structural progressors, defined as those with a loss in medial femorotibial cartilage thickness of (cid:21) 0.10 mm on 3-Tesla MRI and/or a loss in joint-space width [JSW] of (cid:21) 0.20 mm. Associations were assessed using multivariable statistical analyses. Results: Of 400 enrolled participants (mean age 58.6 years; 66.3% women), 374 (93.5%) completed follow-up; 154 (38.5%) lost <5%, 121 (30.2%) lost 5–10%, and 125 (31.2%) lost (cid:21) 10% of body weight. Of these, 93.5%, 71.1% and 21.6% progressed structurally, respectively (P<0.001). Greater weight loss was associated with significantly less cartilage and joint space loss (P<0.001). After adjustment, the odds of structural progression were markedly lower with 5–10% (adjusted odds ratio [aOR] 0.17; 95% CI 0.08–0.36) and (cid:21) 10% weight loss (aOR 0.02; 95% CI 0.01–0.04) versus <5%. Secondary outcomes also improved with greater weight loss (P<0.001). Conclusions: In adults with obesity and knee OA, greater 12-month weight loss was associated with markedly less structural progression, with a clear dose-response gradient and an apparent threshold near 10%. The clinical data presented here support weight-loss magnitude as a clinically-relevant treatment target.
