TL;DR

InBONE Prophecy PSI accurately reproduced planned distance-based positioning, while sagittal talar alignment was more variable.

Abstract

Background

Patient-specific instrumentation (PSI) may improve implant positioning in total ankle arthroplasty (TAA), but three-dimensional validation with postoperative weightbearing CT (WBCT) remains limited. This study assessed the accuracy of the INBONE Prophecy system.

Methods

Fifty-four patients undergoing INBONE II TAA with available Prophecy planning and postoperative WBCT were retrospectively reviewed. Planned and postoperative implant positions were compared for medial tibial component-to-medial malleolus distance, talar stem-to-subtalar joint distance, and talar cut relative to the horizontal axis. Reliability was evaluated using intraclass correlation coefficients.

Results

Distance measurements showed high accuracy, with most cases within 1 mm. Poor accuracy (>2 mm) occurred in 20% of medial malleolar and 7.7% of subtalar measurements. ICCs ranged from 0.79 to 0.95. The mean absolute angular difference for talar cut was 2.91°.

Conclusion

INBONE Prophecy PSI accurately reproduced planned distance-based positioning, while sagittal talar alignment was more variable.

3D-printed patient-specific instrumentation significantly improves the accuracy of prosthesis alignment in unicompartmental knee arthroplasty: a retrospective study

Unicompartmental knee arthroplasty (UKA) is an effective treatment for medial compartment knee osteoarthritis, with its outcomes highly dependent on the accuracy of prosthesis positioning. Conventional UKA, performed using traditional instrumentation, often relies heavily on surgical experience and may lead to variabil...

Accuracy and Precision of Computer-assisted Navigation Compared With Patient-Specific Instrumentation and Conventional Methods in Total Ankle Arthroplasty.

In this laboratory-based TAA study, it is found that both CAN and PSI demonstrated advantages over SRG instrumentation regarding implant placement accuracy and the observed reduction in implant placement variability has potential positive implications for surgeons.

Severe Ankle Deformity Correction Following Transfibular Total Ankle Arthroplasty: A Comparative Cohort Analysis With Minimum 5-Year Follow-Up

Background: There are limited results about total ankle arthroplasty (TAA) in the setting of severe malalignment. The aim of this study was to compare midterm patient-reported and radiographic outcomes in patients with severe varus or valgus tibiotalar deformity treated with primary transfibular TAA at a minimum of 5-y...

Preoperative Medial Proximal Tibial Angle Does Not Influence Clinical Outcomes Following Mobile-Bearing Unicompartmental Knee Arthroplasty.

BACKGROUND The aim of this study was to evaluate whether preoperative medial proximal tibial angle (MPTA) influences clinical outcomes following mobile-bearing medial unicompartmental knee arthroplasty (UKA). METHODS This retrospective study included 454 consecutive patients who had antero-medial osteoarthritis and u...
