S24.0.1. Integrating Anti-CTLA4 Salvage With Locoregional Therapy After Anti-PD-1 in Liver-Dominant Metastatic ESCC

Aug 2026 · Diseases of the esophagus · Vol 39 · 0 citations

TL;DR

Questions relevant to contemporary ESCC management will focus on how to optimally sequence therapies after failure of PD-1-base therapy in advanced or metastatic ESCC, how to incorporate liver-directed strategies for liver-dominant disease or oligoprogression, and after a favorable response to ipilimumab plus nivolumab complicated by severe multisystem irAEs.

Abstract

Esophageal Cancer: Management of Metastatic Cancer The 70-year-old man was diagnosed with squamous cell carcinoma of the mid-esophagus and underwent endoscopic submucosal dissection (ESD) in November 2022. Pathology demonstrated pT1b disease with negative margins but lymphovascular invasion (LVI) present. Given the adverse pathologic feature, he underwent additional lymph node dissection, which confirmed pN0 disease. He was followed with surveillance until September 2024, when he developed local recurrence accompanied by multiple liver metastases, establishing liver-dominant metastatic relapse. First-line systemic therapy was initiated from September to December 2024 with pembrolizumab in combination with paclitaxel, cisplatin, and 5-fluorouracil. Despite treatment, early imaging suggested progressive disease in the liver. He was enrolled into a second-line clinical trial of sacituzumab govitecan, a TROP2-directed antibody drug conjugate, from February to August 2025. Under sacituzumab govitecan, he achieved a radiographic partial response of hepatic lesions. However, subsequent imaging demonstrated liver metastasis progression, without evidence of widespread extrahepatic metastasis. The patient underwent radiofrequency ablation (RFA) of selected liver metastases for local control. Following LRT, subsequent systemic therapy using dual immune checkpoint inhibition with ipilimumab (1 mg/kg) plus nivolumab (3mg/kg). The patient achieved a good radiographic response (partial response, RECIST 1.1). The clinical benefit was achieved, but significant multisystem immune-related adverse events (irAEs) were noted. He developed immune-mediated colitis requiring corticosteroid-based immunosuppression, accompanied by rash. In addition, endocrine irAEs emerged, including hypothyroidism requiring thyroid hormone replacement and adrenal insufficiency, necessitating long-term hormone replacement and close monitoring. The proposed Expert Panel discussion will focus on questions relevant to contemporary ESCC management: (1) how to optimally sequence therapies after failure of PD-1-base therapy in advanced or metastatic ESCC, (2) how to incorporate liver-directed strategies for liver-dominant disease or oligoprogression; and (3) after a favorable response to ipilimumab plus nivolumab complicated by severe multisystem irAEs, what is the most evidence-aligned approach to continuation, de-escalation, or discontinuation with close surveillance.

[Efficacy and safety of sacituzumab govitecan in patients with metastatic HR+/HER2- pleomorphic lobular breast cancer complicated by renal impairment.]

The case concerns a 68-year-old female patient with metastatic HR-positive/HER2-negative pleomorphic lobular breast carcinoma and severe chronic renal impairment since diagnosis, previously treated with multiple lines of therapy. At diagnosis in September 2021, the disease was already metastatic, with bone, nodal, and...

Sacituzumab govitecan combined with anlotinib in metastatic triple negative breast cancer patient with repeated pleural effusion: a case report

Metastatic triple-negative breast cancer (mTNBC) is an aggressive malignancy with limited targeted therapeutic alternatives and dismal long-term prognosis. Sacituzumab govitecan (SG) is a Trop-2-directed antibody-drug conjugate (ADC) conjugated to the topoisomerase I inhibitor SN-38. SG has demonstrated robust anti-tum...

[Use of sacituzumab govitecan in HR+/HER2- metastatic breast cancer following progression on endocrine therapy and chemotherapy: a case report in line with the results of the TROPiCS-02 study.]

Metastatic hormone receptor-positive HR+/HER2- breast cancer represents the most common biological subtype of advanced breast cancer. Despite the significant improvements achieved with the introduction of CDK4/6 inhibitors combined with endocrine therapy, disease progression eventually occurs in a substantial proportio...

Complete pathological response after neoadjuvant chemoimmunotherapy in PD-L1-negative unresectable primary hepatic small cell carcinoma: a case report and tumor microenvironment analysis

Background Primary hepatic small cell carcinoma (PHSCC) is an extraordinarily rare and aggressive malignancy with a dismal prognosis. No standard treatment exists, and the role of immunotherapy in this tumor type, especially in programmed cell death ligand 1 (PD-L1) negative cases, remains unexplored. Case presentation...

Response of HER2 positive gallbladder adenocarcinoma to trastuzumab emtansine (T-DM1): a case report and literature review

The incidence of biliary tract carcinoma (BTC) is gradually increasing, particularly in Asia where it is most common. Compared with other tumors, it has a low incidence but high mortality and is detected at an advanced stage. Metastatic BTCs have fewer options for systemic therapy and limited treatment options beyond s...

HER2-directed therapy with Trastuzumab deruxtecan in recurrent mucinous ovarian carcinoma: complete clinical response and response to rechallenge

Background Mucinous ovarian carcinoma (MOC) is a rare and relatively chemo resistant subtype of epithelial ovarian cancer for which limited evidence-based guidelines exist. Human Epidermal growth factor Receptor 2 (HER2) overexpression occurs in up to 35% of MOC cases; however, targeted therapies are often used off-lab...
