TL;DR

Serum gastric function biomarkers could serve as diagnostic tools for AG, particularly among female patients under 60 years old of age, particularly among female under 60 years old of age.

Abstract

Background While endoscopy is considered the gold standard for diagnosing chronic atrophic gastritis (CAG), serum biomarkers such as pepsinogen I (PG1), pepsinogen Ⅱ (PG2), the PGⅠ/PGⅡ ratio, and gastrin-17 (G-17) have been investigated for their diagnostic value in CAG. However, studies to date have reported inconsistent results regarding their accuracy and clinical utility. Methods All 567 participants underwent upper gastrointestinal endoscopy and serum biomarkers were examined for PG1, PG2 and G-17 using Gastric Function Triple-Parameter test kit (Biohan Biotechnology Co., Ltd). The general characteristics of the participants were derived from the physical examination data from Qilu Hospital of Shandong University. Results Age was identified as a risk factor of CAG (p = 0.01). By incorporating sex and age as covariates into the ROC curve analysis, The serum gastric function biomarkers could serve as diagnostic tools for AG, particularly among female under 60 years old. Within this subgroup, a PG2 cut-off value of ≥6.145 μg/L (AUC = 0.807, sensitivity = 0.762, specificity = 0.800) and a PG1/PG2 ratio cut-off of ≤20.67 (AUC = 0.806, sensitivity = 0.762, specificity = 0.722) showed the strongest discriminatory performance for diagnosing CAG. Meanwhile, PG1 and the PG1/PG2 ratio were significantly associated with the severity of inflammation (p = 0.002 and 0.008, respectively). Conclusions Serum gastric function biomarkers could serve as diagnostic tools for AG, particularly among female patients under 60 years old of age. Meanwhile, PG1 and the PG1/PG2 ratio were significantly associated with the severity of inflammation.

Incremental Diagnostic Value of the Serum Pepsinogen Ratio for Non-Invasive Prediction of Histological Gastric Atrophy: An Internally Validated Clinical–Infectious Model

Background: Histological gastric atrophy represents a critical step in the cascade leading to intestinal-type gastric cancer. Although upper gastrointestinal endoscopy with systematic biopsy remains the reference standard for detecting atrophic changes, its invasiveness, cost and limited suitability for repeated popula...

A Non-Endoscopic Dynamic Nomogram for Atrophic Gastritis Risk Stratification Based on Serum Indicators and Clinical Factors

Background Atrophic gastritis (AG) is a key precancerous lesion in gastric carcinogenesis, underscoring the need for non-endoscopic screening tools to improve early detection. This study aimed to develop a predictive model for AG based on serum indicators and clinical factors. Methods This single-center retrospective s...

Diagnostic Value of Serum HE4, TFF3, and CA72-4 for Gastric Cancer Stratified by Helicobacter pylori Infection Status.

AIMS/BACKGROUND Gastric cancer remains a leading cause of cancer-related mortality worldwide. Helicobacter pylori (Hp) infection is a major risk factor, but the diagnostic performance of serum biomarkers may vary according to Hp infection status. This study aimed to evaluate the diagnostic value of serum human epididym...

Optimal post-gastroscopy timing of the urea [14C] breath test for Helicobacter pylori diagnosis: a histopathology-anchored diagnostic accuracy study

Background Helicobacter pylori is a major cause of gastric carcinogenesis, and accurate detection of active infection guides eradication therapy and cancer prevention. The urea [14C] breath test (UBT) is widely used, but its reliability and optimal timing when performed on the same day after gastroscopy are uncertain....

Diagnostic Performance of Six Tests for Helicobacter pylori Infection in Peptic Ulcer Bleeding: A Prospective Head-to-Head Comparison.

BACKGROUND Rapid urease test (RUT) is widely used for diagnosing Helicobacter pylori (HP) because it is inexpensive and practical. However, its accuracy is reduced in peptic ulcer bleeding (PUB). We prospectively performed a head-to-head comparison of diagnostic tests in the same patients to evaluate their performance...
