Cutaneous cryptococcosis with atypical location in a cat - case report

Abstract

ABSTRACT Cryptococcosis is a rare systemic mycosis among pathogenic fungi, yet it is recognized for its clinical importance in felines. This paper reports on a clinical case of cutaneous cryptococcosis in a domestic cat (Felis catus), presenting a single lesion on the abdominal wall, without ulceration, an uncommon presentation in veterinary literature. The 7-year-old cat had normal laboratory parameters, which contrast with most reports of systemic fungal infections. Diagnosis was based on cytology obtained through fine-needle aspiration (FNA), stained with Panoptic stain and complemented by the India ink technique, highlighting the polysaccharide capsule of Cryptococcus spp. Antifungal therapy with itraconazole and fluconazole, both administered orally, was initiated. Despite early treatment, the cat died of cardiorespiratory arrest on the fourth day of hospitalization. This report emphasizes the importance of early diagnosis, even in atypical clinical presentations, and highlights cytology with specific staining as an accessible and effective diagnostic tool, as well as alerting to the possibility of fulminant disease progression even in the absence of evident clinical and laboratory changes.

A case report of Emergomyces orientalis infection in the lung of HIV infected in patient and literature review

This study reports a rare case of pulmonary Emergomyces orientalis infection with clinical characteristics, diagnostic approaches and therapeutic regimens of this rare fungal infection, aiming to provide evidence for clinical diagnosis and management. We retrospectively analyzed the patient’s clinical symptoms, imaging manifestations, laboratory examinations including metagenomic next-generation sequencing (mNGS), diagnostic and therapeutic procedures, as well as prognosis. Related published literature was searched, reviewed and summarized. The patient presented with a 3-day cough and expectoration. Conventional pathogen examinations showed the patient is infected with the HIV virus and definite diagnosis of Emergomyces orientalis infection was finally confirmed by mNGS of lung biopsy specimens. He received liposomal amphotericin B combined with symptomatic supportive therapy. Clinical symptoms were alleviated and pulmonary lesions were absorbed after treatment. Pulmonary Emergomyces orientalis infection is uncommon and prone to missed diagnosis and misdiagnosis with high mortality. Routine pathogen culture yields a low positive rate. mNGS serves as a critical tool for early and accurate diagnosis. Individualized antifungal treatment, strict drug interaction assessment and therapeutic monitoring are essential to optimize clinical prognosis.

Cutaneous Aspergillosis Concurrent with Subclinical Parasitism In a 4-month-old Red Sokoto Kid: a Case Report

Aspergillosis is a fungal disease affecting a wide range of animal species with varying clinical manifestations. A 4-month-old Red Sokoto kid was presented with a history of generalized skin lesions and diarrhea. On physical examination, nodular swellings, crusty lesions, alopecia, and diarrhea were observed. Samples were taken to microbiology, parasitology, and clinical pathology laboratories. Aspergillus spp., Eimeria, and Strongyloides, as well as neutrophilia, monocytosis, and eosinophilia, were observed in the laboratory results. The condition was presumptively diagnosed as cutaneous aspergillosis concurrent with gastrointestinal parasitism. Treatment included topical antifungals, anthelminthics, and broad-spectrum antibiotics, and the kid responded positively to treatment. Based on the findings of this case, it was concluded that cutaneous aspergillosis, though rare, should be considered in goats presenting with chronic or atypical skin lesions, particularly when accompanied by concurrent parasitism. It was recommended that good husbandry practices, regular parasite control, and early veterinary intervention be emphasized to prevent disease occurrence.

Intestinal criptococosis in a dog - case report

ABSTRACT Cryptococcosis is a fungal infection that most commonly affects the respiratory tract or central nervous system. However, dogs and cats may also develop cutaneous and gastrointestinal forms of the disease, although these are less frequently reported. This article describes a rare case of intestinal cryptococcosis in a dog. The patient was referred for evaluation due to intermittent bilious vomiting, diarrhea, weight loss, and hyporexia. Physical examination revealed mild hyperthermia and a palpable abdominal mass. Abdominal ultrasonography identified mesenteric lymphadenopathy and alterations in the intestinal loops. Based on these findings, the patient underwent exploratory celiotomy, which revealed an enlarged and reactive mesenteric lymph node. Cytological analysis of fine-needle aspirates from the lymph node demonstrated fungal structures, and PCR confirmed infection by Cryptococcus spp. Histopathological examination of mesenteric lymph node and intestinal tissue samples revealed granulomatous inflammation with areas of necrosis and fungal hyphae. These findings confirmed the rare diagnosis of granulomatous intestinal cryptococcosis in a dog.

From Suppurative Inflammation to Fibrotic Remodeling in Cutaneous Nocardiopsis Infection.

In the present case report, a cutaneous infection caused by the rare actinomycete Nocardiopsis sp. TSRI0078 is described. A 77-year-old woman with a medical history of hypertension and diabetes presented with ulcerated purplish-red nodules on the lower extremities that had persisted for 3 months. A histopathological examination revealed infectious granulomas predominantly composed of neutrophils. After empirical antibiotic therapy proved ineffective, the pathogen was confirmed to be Nocardiopsis sp. TSRI0078 via polymerase chain reaction testing, gene sequencing, and microbial culture. Subsequent treatment with oral trimethoprim-sulfamethoxazole led to gradual regression of the lesions and scar formation. With only 11 human infection cases reported globally, a clear understanding of the clinical manifestations of Nocardiopsis infections is lacking. This pathogen should be considered in the differential diagnosis of chronic, unexplained cutaneous lesions, particularly in patients with underlying diseases. A combination of molecular and conventional microbiological examinations facilitates early diagnosis, and trimethoprim-sulfamethoxazole may represent an effective therapeutic option.
