In a nationwide contemporary PCI population, clopidogrel monotherapy was associated with lower ischemic risk, primarily driven by fewer MIs, with no difference in overall bleeding events despite a lower risk of ICH.

Abstract

Optimal single antiplatelet therapy (SAPT) during the chronic maintenance period after coronary stenting remains uncertain. Although HOST-EXAM demonstrated superiority of clopidogrel over aspirin, real-world evidence remains limited. Using the Korean Health Insurance Review and Assessment Service (HIRA) claims database, we identified 49,227 patients who completed 6–24 months of dual antiplatelet therapy after percutaneous coronary intervention (PCI) and then received SAPT (aspirin, n = 20,817; clopidogrel, n = 28,410). The efficacy outcome was a composite of myocardial infarction (MI) and ischemic stroke; the safety outcome was a composite of gastrointestinal (GI) bleeding and intracranial hemorrhage (ICH). Propensity score weighting was used to balance covariates between groups. Compared with aspirin, clopidogrel was associated with lower risks of MI (hazard ratio [HR] 0.88; 95% confidence interval [CI] 0.81–0.97) and the efficacy composite (HR 0.93; 95% CI 0.88–0.98). Risks of ischemic stroke and gastrointestinal bleeding were similar. Clopidogrel was associated with a lower risk of ICH (HR 0.81; 95% CI 0.69–0.94), while the overall safety composite did not differ (HR 1.00; 95% CI 0.92–1.08). In a nationwide contemporary PCI population, clopidogrel monotherapy was associated with lower ischemic risk, primarily driven by fewer MIs, with no difference in overall bleeding events despite a lower risk of ICH.

OPTIMAL DURATION OF DUAL ANTIPLATELET THERAPY AFTER PERCUTANEOUS CORONARY INTERVENTION: A NETWORK META-ANALYSIS OF RANDOMIZED CONTROLLED TRIALS

Coronary artery disease is commonly managed with percutaneous coronary intervention (PCI) followed by dual antiplatelet therapy (DAPT). However, bleeding risk has driven interest in shorter DAPT durations. This network meta-analysis evaluates the efficacy and safety of different DAPT durations after PCI. P...

P2Y12 Inhibitor Selection and Ischemic Outcomes in Patients with Acute Coronary Syndrome Undergoing Percutaneous Coronary Intervention: A Real-World Registry Analysis

This study evaluated whether ticagrelor use is associated with improved ischemic outcomes compared with clopidogrel in Korean patients with acute coronary syndrome undergoing percutaneous coronary intervention. We analyzed patient-level data from 1,199 patients (614 clopidogrel, 585 ticagrelor) enrolled in two prospect...

Outcome evaluation of percutaneous coronary intervention vs medical therapy alone in patients with chronic coronary syndrome

Among CCS patients diagnosed non-invasively, the medication-only group demonstrated comparable all-cause mortality and MI rates, but a significantly lower risk of revascularization compared with the PCI group, while incurring only onethird of the healthcare costs.

Low-dose prasugrel versus clopidogrel for dual antiplatelet therapy after percutaneous coronary intervention: insights from the STOPDAPT-2 total cohort.

No significant differences were detected in short-term cardiovascular or bleeding outcomes between low-dose prasugrel-based and clopidogrel-based DAPT in patients after PCI without major in-hospital complications.

Clopidogrel or Dual Antiplatelet Therapy in High-Ischemic-Risk Patients.

Among patients at high risk for ischemic events 12 months after drug-eluting stent implantation, clopidogrel monotherapy was noninferior to extended DAPT with respect to net adverse clinical events at 24 months.

Real world outcomes of aspirin monotherapy compared with aspirin plus clopidogrel in a south Indian cohort with cardiovascular and cerebrovascular disease

Cardiovascular and cerebrovascular diseases remain leading causes of global morbidity and mortality, with platelet activation playing a central pathogenic role. Aspirin is the cornerstone of antiplatelet therapy, while dual antiplatelet therapy (DAPT) with aspirin and clopidogrel offers enhanced platelet inhibition in...
