A risk assessment method for ESCC using buccal mucosa is established, which may contribute to early detection of ESCC.

Abstract

Esophageal Cancer: Esophageal Carcinogenesis The oral cavity, pharynx, and esophagus are covered by anatomically contiguous squamous epithelium, and alcohol consumption and smoking are well-established common cancer risk factors for the three organs. The aim of this study is to analyze somatic mosaicism in normal buccal mucosa and develop a predictive model for esophageal squamous cell carcinoma (ESCC) based on buccal mucosal remodeling. We analyzed ESCC driver mutant clones using error-corrected sequencing of buccal swabs and examined the association between driver mutations and clinical factors. A total of 707 buccal swabs of three different sizes (20, 28, and 254 mm2) from 222 subjects (121 ESCC patients and 101 controls) were analyzed, resulting in the detection of 23,537 mutations, with frequencies of NOTCH1, TP53, and CHEK2. Aging and smoking had a consistent impact on the accumulation of driver mutations regardless of germline risk. In contrast, alcohol consumption led to approximately 20-fold faster accumulation of driver mutations in individuals with germline risk compared to those without, even at the same level of alcohol intake. Logistic regression analysis was used to construct a prediction model for ESCC based on buccal mucosal remodeling, with the most accurate model achieving an AUC of 0.85 using 28 mm2 buccal swabs. In conclusion, we established a risk assessment method for ESCC using buccal mucosa, which may contribute to early detection of ESCC.

International case-control study of DNA methylation biomarkers for the minimally invasive diagnosis of esophageal squamous cell carcinoma.

BACKGROUND Esophageal squamous cell carcinoma (ESCC), the most common type of esophageal cancer worldwide, is usually diagnosed via esophagogastroduodenoscopy (EGD), an expensive procedure having limited availability in low- and Middle-Income countries (LMICs). We therefore developed diagnostic strategy that applies ES...

Epidemiological risk factors and inferred p53 pathway dysregulation in Esophageal squamous cell carcinoma in the Somali population

Esophageal squamous cell carcinoma (ESCC) is the most prevalent cancer in Somalia and constitutes a major public health concern in the Horn of Africa. Despite its high incidence, the molecular mechanisms underlying ESCC are not well characterized. This scoping review synthesizes published data on TP53 mutations, enviro...

Metagenomic Analysis of the Oral Microbiome in Patients with Head and Neck Squamous Cell Carcinoma: A Case-Control Study

Head and neck squamous cell carcinoma (HNSCC) is one of the important cancers that are responsible for considerable morbidity all over the world. Recent studies reveal that changes in the oral microbiome are linked with HNSCC, but the exact microbial signatures according to the presence of disease need further research...

Liquid Biopsy and Nonendoscopic Biomarkers for Precision Early Detection of Esophageal Cancer: Evidence, Risk Stratification and Translational Challenges

Esophageal cancer remains one of the most lethal malignancies worldwide, largely because most tumors are detected after symptoms develop. Esophageal squamous cell carcinoma (ESCC) and esophageal adenocarcinoma (EAC) differ in epidemiology, precursor lesions and molecular biology, and therefore require subtype-specific...

Lesion-specific oral microbiome signatures and predicted carcinogenic pathways in oral squamous cell carcinoma: a paired-site study in Pakistan

ABSTRACT Background Oral squamous cell carcinoma accounts for over 90% of oral neoplasms. Despite therapeutic advances, the lack of reliable, non-invasive biomarkers and delayed diagnosis continues to impede effective clinical management. By combining paired lesion and non-lesion sampling with predictive metagenomics a...
