The inpatient economic burden of hospitalized patients with Puumala virus (PUUV) infections in Styria, Austria’s primary endemic region, is quantified to identify hospitalization costs and early admission predictors of costs.

Abstract

Abstract Background Hantavirus infections cause hemorrhagic fever with renal syndrome and impose a potential burden on healthcare systems. However, comprehensive economic data remain scarce. We quantified the inpatient economic burden of hospitalized patients with Puumala virus (PUUV) infections in Styria, Austria’s primary endemic region, to identify hospitalization costs and early admission predictors of costs. Methods This retrospective cost-of-illness study analyzed 1049 serologically confirmed PUUV hospitalizations across 20 public hospitals in Styria, Austria between September 2003 and March 2024. Costs were estimated using the Austrian performance-oriented hospital financing (Leistungsorientierte Krankenanstaltenfinanzierung) diagnosis-related group system, standardized to 2024-equivalent Euros using consumer price index deflation. Admission predictors of cost were assessed using multivariable γ-generalized linear modeling with multiple imputations. Temporal trends in length of stay were analyzed using weighted segmented linear regression. Results Cumulative inpatient cost was ∼€10.3 million (mean per-case: €9788) for the entire study period of 21 years. Three epidemic years (2012, 2019, and 2021) accounted for 53.6% of cases and over €5.2 million in expenditure, with peak annual costs reaching €1.93 million (0.274% of total Styrian inpatient expenditure) in 2012. Prolonged activated partial thromboplastin time (aPTT; cost-ratio 1.024 per second, 95% CI 1.019–1.030) and older age (CR 1.004 per year, 95% CI 1.001–1.008) were independent admission-level predictors of higher cost. Median hospital length of stay declined by ∼50% over the study period. Conclusions PUUV hospitalizations impose a sharply episodic but recurrent economic burden on regional healthcare systems. Admission aPTT and age independently predicted cost, but clinical outcomes were not modeled and these variables do not constitute a triage tool. Epidemic preparedness strategies are warranted.

Trends and estimated costs of hospitalizations potentially associated with respiratory syncytial virus among children in Brazil, 2014-2025

The association between prolonged hospitalization and higher expenditures underscores the need for targeted, region-specific preventive strategies and timely clinical interventions, to reduce the growing economic burden on the system.

Burden of typhoid fever in primary and hospital care in Indonesia: a serial cross-sectional study using national health insurance (BPJS Kesehatan) data, 2015–2023

Summary Background Despite Indonesia's high typhoid burden, subnational prevalence data remain sparse, limiting risk mapping and targeted interventions. We characterised trends in typhoid presentation in primary care and hospital settings, described geographic patterns, and identified factors associated with hospitalis...

Nationwide Q Fever Hospitalizations in Italy, 2014–2023: Evidence of Underrecognition and a Sardinian Hotspot

Abstract Background Q fever is a zoonotic disease caused by Coxiella burnetii. In Italy, serologic evidence shows exposure in humans and livestock, but surveillance data are sparse, and underreporting is likely. We aimed to characterize Q fever-related hospitalizations in Italy during 2014–2023 and examine environmenta...

Healthcare resource utilization and costs associated with a diagnosis of infectious mononucleosis in children, adolescents, and adults in the United States

Abstract Objective Epstein-Barr virus (EBV) is responsible for 90% of infectious mononucleosis (IM) cases. Clinically, IM often presents with fever, fatigue, sore throat, and lymphadenopathy, but can also result in prolonged symptoms requiring hospitalization. As little is known about the impact of IM on healthcare res...

Clostridioides difficile Infection in a Romanian Tertiary Care Hospital: Epidemiology, Comorbidity Burden, Treatment Patterns, and Mortality Outcomes (2023–2025)

Background: Real-world outcome data on Clostridioides difficile infection (CDI) from Eastern Europe are scarce, and partly digitised hospital records make surveillance laborious. We characterised the epidemiology, comorbidity, treatment and in-hospital mortality of CDI in a Romanian tertiary hospital and validated a ru...

Amebic Liver Abscess Hospitalizations in the United States: A Nationwide Analysis of Trends and Severe Morbidity, 2002-2023.

AIMS US population-based data on amebic liver abscess (ALA) are limited and predate the ICD-10-CM era. We quantified national trends, inpatient outcomes, and factors associated with severe in-hospital morbidity among adult ALA hospitalizations.
