TL;DR

A 4-year-old boy with dextrocardia, congenitally corrected transposition of the great arteries, pulmonary valve stenosis, and ventricular septal defect demonstrated progressive hypoxemia and mild-to-moderate mitral and tricuspid regurgitation, allowing for definitive anatomic repair at 4 years and 7 months of age.

Abstract

A 4-year-old boy with dextrocardia, congenitally corrected transposition of the great arteries, pulmonary valve stenosis, and ventricular septal defect demonstrated progressive hypoxemia and mild-to-moderate mitral and tricuspid regurgitation. Cardiac magnetic resonance imaging revealed insufficient right ventricular volume for Senning-Rastelli type anatomical repair. Given the elevated surgical risk associated with mitral regurgitation and inadequate ventricular volume, an initial palliative approach was undertaken at 2 years of age, consisting of systemic-to-pulmonary shunt placement and mitral valve repair, which improved oxygenation and promoted right ventricular growth, with trivial mitral regurgitation. Subsequent imaging confirmed adequate ventricular development and trivial mitral regurgitation, allowing for definitive anatomic repair at 4 years and 7 months of age. The procedure comprised a Senning atrial switch combined with a Rastelli operation. The postoperative course was uneventful, and imaging demonstrated preserved biventricular function with minimal atrioventricular valve regurgitation and no evidence of outflow or pathway obstruction.

A combined valve-sparing aortic root replacement and primary tetralogy of Fallot repair in an unoperated adult: a case report

This case demonstrates the feasibility of combining Yacoub VSARR with primary intracardiac TOF repair in unoperated adults with TOF-associated aortopathy, and may expand surgical options for adults with TOF-associated aortopathy, though long-term durability data are needed.

Mitral Valve Transplantation: a valuable option?

OBJECTIVE Partial heart transplantation (PHT) offers a growth-accommodating alternative to mechanical valve replacement in congenital heart disease. While experience with semilunar valve PHT is expanding, application to atrioventricular valves remains extremely limited. We report the first mitral valve PHT at our insti...

Isolated Double-Orifice Mitral Valve With Severe Mitral Regurgitation: Successful Mitral Valve Repair.

BACKGROUND Severe mitral regurgitation (MR) associated with isolated double-orifice mitral valve (DOMV) in adults is uncommon, and both its diagnosis and surgical management remain challenging. CASE SUMMARY A 60-year-old woman presented with symptomatic severe MR and was scheduled for mitral valve repair. Transthorac...

Transjugular LuX-Valve Plus transcatheter tricuspid valve replacement for torrential tricuspid regurgitation in a patient with a giant right atrium and multiple prior valve interventions: a case report

Abstract Background Transcatheter tricuspid valve replacement (TTVR) is an option for selected patients with severe tricuspid regurgitation (TR) who are at high surgical risk, but experience remains limited in those with extreme right-heart anatomy, including a giant right atrium and a markedly enlarged elliptical tric...

Patent Ductus Arteriosus Recanalization in Tricuspid Atresia with Restrictive Ventricular Septal Defect and Moderate Pulmonary Valve Stenosis: A Minimally Invasive Strategy in a Critical Case

PDA recanalization with stent implantation is a viable and life-saving minimally invasive option in critically ill patients with duct-dependent CHD when pharmacologic support fails and may serve as a bridge to staged surgical palliation.
