This case demonstrates the value of CT in defining lesion extent and guiding surgical planning and supports aggressive surgical management despite severe concurrent pulmonary disease.

Abstract

An 8‐year‐old, entire, male German shorthaired pointer presented with acute respiratory distress, pyrexia and coughing following a period of boarding. Thoracic radiographs identified a caudal mediastinal mass effect with severe multilobar bronchopneumonia. Computed tomography (CT) further characterised an encapsulated, peripherally enhancing, fluid‐ and gas‐filled lesion within the caudal mediastinum, closely associated with the right caudal lung lobe. Median sternotomy was performed for abscess drainage and resection of the right caudal and accessory lung lobes due to extensive fibrous adhesions. Anaerobic culture yielded Bacteroides pyogenes , and histopathology confirmed chronic neutrophilic pneumonia and pleuritis without evidence of neoplasia or foreign material. The dog recovered uneventfully and was clinically normal at 7 weeks postoperatively. This case demonstrates the value of CT in defining lesion extent and guiding surgical planning and supports aggressive surgical management despite severe concurrent pulmonary disease.

A case of Lemierre-like syndrome with mediastinal-pulmonary communication managed without lung resection.

Lemierre-like syndrome, an atypical variant of Lemierre's syndrome, is characterised by septic thrombophlebitis without classical features and may be complicated by descending necrotising mediastinitis (DNM). Contrast-enhanced computed tomography revealed extensive thrombosis of the right internal jugular, external jug...

Thoracotomy for Urgent Repair of Lung Torsion Identified as an Incidental Finding

We report a 68-year-old male with chronic obstructive pulmonary disease on home oxygen who presented with foot swelling and erythema, associated with a new right foot wound. He denied chest pain and overt shortness of breath at the time of his presentation. Initial chest radiography obtained during the evaluation raise...

A rare cause of pneumomediastinum and descending necrotizing mediastinitis: Untreated odontogenic abscess

Pneumomediastinum is the presence of free air within the mediastinum and may be primary or secondary. Secondary pneumomediastinum typically follows trauma, invasive procedures, mechanical ventilation, or dental interventions using pressurized air; odontogenic causes without prior intervention are exceptionally rare...

Adult extralobar pulmonary sequestration presenting as a posterior mediastinal mass and hemothorax: a report of two cases

Pulmonary sequestration is a rare congenital malformation characterized by dysplastic lung tissue lacking communication with the tracheobronchial tree and receiving systemic arterial supply from aberrant vessels. Extralobar pulmonary sequestration can present along a broad clinical spectrum, ranging from an incid...
