Abstract
Introduction. Autoimmune thyroiditis has a leading place in the structure of thyroid pathology; its pathogenesis is based on the onset of an immunopathological process directed against thyroid antigens, leading to persistent hypothyroidism. Not always normalization of the hormonal background with thyroid hormone preparations and conventional means for correcting violations of immunological and neurological indicators lead to an improvement in the condition of this category of patients. Since one of actual goals of restorative medicine is implementing highly medically and economically effective non-drug methods for increasing the functional reserves of the body, normobaric hypoxytherapy can be used in the treatment of this disease, since the literature has accumulated many years of experience in the treatment of various diseases by hypoxytherapy. Purpose. A clinical and laboratory assessment of the effectiveness of hypoxytherapy in the correction of neuroendocrinoimmune disorders in children and adolescents with hypothyroidism due to autoimmune thyroiditis. Materials and methods. 145 children and adolescents aged 6–16 years diagnosed with autoimmune thyroiditis in the stage of hypothyroidism underwent a course of hypoxytherapy against the background of hormone therapy; 105 children and adolescents entered the control group. The hypoxic mixture was fed from a hypoxicator converting room air into an oxygen-controlled hypoxic mixture. The autoimmune nature of the disease was confirmed based on an increased level of antibodies against thyroperoxidase and thyroglobulin. To assess cognitive impairment, changes in attention were determined (a correction test according to the Anfimov table). The level of wakefulness was determined by omegametry. Results. The interval hypoxic training led to the normalization of the content of thyroid hormones in the blood, as the level of triiodothyronine increased from 2.3±0.02 to 5.5±0.01 pmol/L, the content of thyroxine increased from 6.1±0.03 to 16.4±0.01 pmol/L, and the production of thyroid-stimulating pituitary hormone decreased from 7.8±0.04 to 2.6±0.05 μIU/ml. An increase in the function and number of CD8+cells from 13.9±0.47 to 29.6±1.18% after a course of hypoxytherapy prevents the progression of the autoimmune process and contributes to the restoration of the hormonal process, which in turn leads to positive trends in the neurological status of patients, improved mental performance, and fine coordination of movements: аaccording to the results of the Anfimov’s correction test, the mental productivity coefficient increased in children from 483.6±12.1 to 873.6±17.4, and in adolescents from 712.0±18.3 to 1119.6±24.7; the time for passing the Thorndike labyrinth decreased in children from 110±4 to 80±2 s, in adolescents from 100±5 to 65±4 s. There were no complications or deterioration of the patient’s conditions associated with the therapy. Conclusion. The therapeutic effect of hypoxytherapy is realized not only through the strengthening of compensatory mechanisms ensuring the delivery of oxygen to the tissues, but also through the inhibition of humoral immune reactions and stimulation of the T-cell link of immunity in patients with autoimmune thyroiditis. Positive hormonal-immunological and neurological changes in children and adolescents with autoimmune thyroiditis after hypoxytherapy indicates the advisability of its inclusion in the pathogenetic therapy regimens for patients with autoimmune thyroiditis.
