TL;DR

Compared with JWT, BSKT may provide more favorable side-branch protection and selected short-term functional benefits in patients with LAD-diagonal branch true bifurcation lesions, but its long-term clinical benefit remains uncertain.

Abstract

Objectives: To compare the clinical efficacy of the Balloon Stent Kissing Technique (BSKT) and Junctional Wire Technique (JWT) for treating true bifurcation lesions of the coronary left anterior descending artery (LAD) and diagonal branch. Methodology: A retrospective analysis included data from 235 patients who underwent treatment of true bifurcation lesions of the coronary LAD and diagonal branch at Tianjin University Chest Hospital between February 2023 to October 2024. All patients were followed up for one year. Based on the technique, patients were divided into the BSKT (n=111) and the JWT (n=124) groups. Efficacy, safety profiles, and myocardial perfusion parameters were compared between the two groups. Results: In the unadjusted analysis, BSKT was associated with a higher immediate stent implantation success rate and a lower incidence of provisional stenting (PS-T) due to branch compromise. After adjustment for potential confounders, the association with immediate stent implantation success was attenuated and became borderline significant. Prognostic outcomes were comparable between the two groups (P > 0.05). On postoperative Day 1, BNP and cTnT levels remained lower in the BSKT group after correction for repeated longitudinal comparisons, whereas the difference in CK-MB was attenuated after multiple-comparison correction. At six months, the BSKT group showed more favorable selected SPECT-derived parameters, including a lower stress myocardial perfusion score, fewer resting and stress myocardial ischemic segments, and a narrower range of abnormal myocardial perfusion, whereas the resting myocardial perfusion score did not differ significantly between groups. Conclusion: Compared with JWT, BSKT may provide more favorable side-branch protection and selected short-term functional benefits in patients with LAD-diagonal branch true bifurcation lesions, but its long-term clinical benefit remains uncertain.

One-year outcomes after hybrid DES-DCB and two-stent strategies for true distal left main bifurcation lesions: A single-center registry.

BACKGROUND True distal left main (LM) bifurcation percutaneous coronary intervention (PCI) is technically challenging. A hybrid strategy using a drug-eluting stent (DES) in the main branch and a drug-coated balloon (DCB) in the side branch may reduce permanent metal at the bifurcation, but LM-specific data remain limit...

Clinical and procedural outcomes of ostial left anterior descending artery intervention with or without crossover to left-main

It remains debated whether just ostial stenting (JOS) or crossover stenting (COS) is optimal for isolated ostial lesions of the left anterior descending (LAD) artery, and available comparative data on intravascular imaging (IVI)-guided percutaneous coronary intervention (PCI) remains limited. We aim to compare clinical...

Long-Term Outcomes According to the Initially Intended Stenting Strategy for Unprotected Distal Left Main Bifurcation PCI: A Single-Centre Retrospective Cohort Study

Background: Optimal stenting for distal unprotected left main coronary artery (LMCA) bifurcation disease remains uncertain. We evaluated long-term outcomes by initially intended strategy. Methods: This retrospective, single-centre cohort included 234 consecutive patients undergoing percutaneous coronary intervention (P...

Stenting Technique and Clinical Outcome in True Bifurcation PCI: Insights From the OCTOBER Trial.

BACKGROUND Optimal stenting for true coronary bifurcation lesions is widely debated, with few studies using predefined intracoronary imaging. OBJECTIVES The aim of this study was to assess predictors and outcomes associated with a 1 vs a 2-stent technique and whether optical coherence tomographic (OCT) guidance modif...

Hybrid strategy for percutaneous coronary intervention in diffuse left coronary artery disease: single-center one-year outcomes

The treatment of diffuse coronary artery disease remains technically demanding due to long lesions, distal vessel tapering, and side branches, and may require extensive metal implantation. A segment-oriented hybrid PCI approach combines the predictable mechanical support of a drug-eluting stent in the proximal vessel s...

Mini-crush versus provisional stenting in coronary bifurcation lesions: long-term results from a real-world cohort.

BACKGROUND The optimal stenting strategy for coronary bifurcation lesions remains a topic of debate, particularly regarding the long-term comparative effectiveness of mini-crush and provisional techniques. METHODS We analysed consecutive patients undergoing bifurcation percutaneous coronary intervention (PCI) treated...
