Bedside EIT enables dynamic assessment of pulmonary status and may assist in optimizing ventilator settings, providing a useful reference for the management of FES complicated by severe ARDS.

Abstract

Background Fat embolism syndrome (FES) is a rare complication of severe multiple trauma. Electrical impedance tomography (EIT) is a noninvasive, real-time, bedside tool for monitoring regional pulmonary ventilation. Veno-venous extracorporeal membrane oxygenation (VV-ECMO) may improve outcomes in critically ill patients in whom conventional mechanical ventilation fails. Ultra-protective lung ventilation strategy (ULPVS) uses very low tidal volumes to markedly reduce lung stress and mitigate ventilator-induced lung injury (VILI). Case presentation In June 2025, a 57-year-old woman was admitted to the Department of Orthopedics with a femoral shaft fracture following a motor vehicle accident. Severe respiratory distress developed postoperatively, and she was urgently transferred to the ICU. Based on laboratory findings, chest CT findings, and Oil Red O staining of bronchoalveolar lavage fluid (BALF), FES complicated by severe acute respiratory distress syndrome (ARDS) was considered. In addition to conventional treatment, VV-ECMO was initiated to provide gas-exchange support. An ULPVS was implemented during the early phase of ventilation to reduce the risk of VILI, and EIT was used to guide ventilatory management and titration of the optimal Positive end-expiratory pressure (PEEP) Following these interventions, static lung compliance increased from 46 mL/cmH2O to 67.1 mL/cmH2O. VV-ECMO support lasted 62 h, and the total duration of mechanical ventilation was 113 h. After stabilization, the patient was transferred back to the Department of Orthopedics and was ultimately discharged in good condition. Conclusion Bedside EIT enables dynamic assessment of pulmonary status and may assist in optimizing ventilator settings, providing a useful reference for the management of FES complicated by severe ARDS.

Electrical Impedance Tomography-Guided Precision Nursing in an Older Patient With Septic Shock and ARDS: A Case Report.

Septic shock complicated by acute respiratory distress syndrome (ARDS) and multiple organ dysfunction syndrome (MODS) presents a fundamental therapeutic conflict: shock demands aggressive fluid resuscitation, whereas lung protection requires fluid restriction and a negative fluid balance. We report the precision nursin...

Electrical impedance tomography–guided individualized ventilation strategy in patients with trauma-related and postoperative acute respiratory distress syndrome

Objective Optimal positive end-expiratory pressure (PEEP) selection in acute respiratory distress syndrome (ARDS) is challenging because of marked regional lung heterogeneity. Electrical impedance tomography (EIT) provides bedside assessment of regional ventilation and may support individualized ventilation by informin...

Case Report: Pulmonary reperfusion injury with bilateral bronchial cast thrombosis after VA-ECMO-bridged catheter-based reperfusion for cardiac arrest caused by high-risk pulmonary embolism

High-risk pulmonary embolism (PE) can lead to acute right ventricular failure, obstructive shock, and cardiac arrest. Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) provides bridging life support, buying time for imaging confirmation and reperfusion. However, reperfusion injury, pulmonary hemorrhage, and a...

Heparin-free veno-venous extracorporeal membrane oxygenation as a bridge therapy for airway interventions in malignant central airway obstruction

Rationale: Malignant central airway obstruction (MCAO) represents a life-threatening oncologic emergency that can lead to rapid respiratory decompensation and death. Traditional management with mechanical ventilation may be inadequate or risky when severe airway narrowing or complete obstruction exists. Veno-venous ext...

Case Report: Postoperative pulmonary embolism causing obstructive shock and circulatory collapse after gynecologic surgery: successful rescue with VA-ECMO

Very-high-risk pulmonary embolism (PE) occurring within hours after major surgery presents a therapeutic dilemma because urgent reperfusion may be required at precisely the time when bleeding risk is greatest. We describe a 45-year-old woman who developed catastrophic bilateral PE approximately two hours after elective...

Electrical impedance tomography assessment of bronchoalveolar lavage-associated lung derecruitment and its related factors: a single-center retrospective study

Background Bronchoalveolar lavage (BAL) may cause regional lung derecruitment during bronchoscopy. However, this effect may be underestimated by conventional oxygenation monitoring, especially under general anesthesia and mechanical ventilation. This study aimed to assess BAL-associated lung derecruitment using electri...
