TL;DR

Tibial artery Doppler measurements reflect peripheral vasoconstriction and may indicate fetal adaptation to hypoxia in FGR and Tibial-placental ratio demonstrates potential as a non-invasive tool for detecting fetal compromise.

Abstract

ObjectivesTo evaluate the role of tibial artery pulsatility index (TA-PI) and tibial-placental ratio (TPR) in distinguishing fetal growth restriction (FGR) from normal pregnancies and to assess their predictive value for adverse neonatal outcomes.MethodsThis prospective observational cohort study was conducted at a tertiary perinatology clinic between 2024 and 2025. A total of 42 pregnant women at 37 weeks of gestation were enrolled and categorized into two groups: FGR (n = 21) and non-FGR controls (n = 21). Doppler ultrasonography was used to assess middle cerebral artery, umbilical artery, cerebroplacental ratio, and tibial artery parameters. Statistical analyses included t-tests, chi-square tests, and receiver operating characteristic curve analysis to evaluate the diagnostic performance of TA-PI and TPR.ResultsTA-PI was significantly higher in the FGR group, indicating increased peripheral vascular resistance, while peak systolic velocity in the tibial artery was significantly lower, suggesting compromised peripheral circulation. Both cerebroplacental and tibial-placental ratios were elevated in FGR fetuses, indicating circulatory redistribution. Tibial-placental ratio showed strong predictive value for distinguishing FGR from normal pregnancies. Neonates with FGR had significantly lower birth weights and lower APGAR scores at one and five minutes. Although neonatal intensive care unit admissions were more frequent in the FGR group, the difference was not statistically significant.ConclusionsTibial artery Doppler measurements reflect peripheral vasoconstriction and may indicate fetal adaptation to hypoxia in FGR. Tibial-placental ratio demonstrates potential as a non-invasive tool for detecting fetal compromise.Advances in knowledgeThis study introduces tibial artery Doppler parameters as promising indicators of fetal hemodynamic adaptation, offering a novel approach to enhancing non-invasive FGR surveillance.

Association between uterine artery Doppler velocimetry, placenta accreta spectrum, and depth of placental implantation

Abstract Introduction We aim to evaluate uterine artery (UtA) Doppler velocimetry in patients at risk of placenta accreta spectrum (PAS). Methods Prospective cohort of patients referred to an academic referral center for suspected PAS. The left and right UtA were evaluated, and mean peak systolic velocity (PSV), end‐di...

Assessing fetal circulatory alterations in preeclampsia: The role of caval aortic index and left brachiocephalic vein diameter.

OBJECTIVE The primary objective of this study is to compare the fetal caval aortic (IVC/Ao) index and left brachiocephalic vein (LBCV) diameter between pregnancies with preeclampsia (PE) and healthy controls. The study also assesses their association with disease severity and perinatal outcomes. METHODS In this prosp...

Evaluation of the Fetal Myocardial Performance Index at Mid-Gestation for Prediction of Fetal Growth Restriction.

OBJECTIVE Fetal growth restriction (FGR) is associated with increased perinatal morbidity and mortality. The fetal myocardial performance index (MPI) has been proposed as a marker of fetal cardiovascular adaptation for FGR; thus, this study aims to evaluate whether fetal MPI measured at 20-24 gestational weeks predicts...

Increasing Trends of Pregnancy-Induced Hypertension: Need for Uterine Artery Doppler as a Predictor

Background: Pregnancy-induced hypertension (PIH) remains a leading cause of maternal and perinatal morbidity and mortality worldwide, with increasing incidence noted in developing countries. Early identification of at-risk pregnancies is crucial for timely intervention. Uterine artery Doppler (UAD) velocimetry offers a...

Association of fetal venous diameter and volume flow measurements with late-onset fetal growth restriction: A prospective case-control study.

OBJECTIVE This study investigates the diagnostic utility of venous diameter (d) and volume flow (Q) measurements of the umbilical vein (UmbV), ductus venosus (DV), inferior vena cava (IVC), and subdiaphragmatic vestibulum (SDV) in fetuses with late-onset fetal growth restriction (FGR).
