TL;DR

This narrative review synthesizes current evidence on the diagnostic potential of stem cell-associated miRNAs in bone-related pathologies, and underscores the promise of stem cell-associated miRNAs as diagnostic tools in osteogenic disorders, while identifying critical gaps that must be addressed to enable their incorporation into precision medicine strategies.

Abstract

Stem cell-associated microRNAs (miRNAs) have emerged as pivotal regulators of osteogenesis, influencing lineage commitment, differentiation, and skeletal tissue homeostasis. Their dysregulation has been increasingly linked to a spectrum of osteogenic disorders, including osteoporosis, osteoarthritis, and impaired fracture healing. This narrative review synthesizes current evidence on the diagnostic potential of stem cell-associated miRNAs in bone-related pathologies. We highlight their roles in modulating osteoblast and osteoclast activity, maintaining mesenchymal stem cell fate, and orchestrating signaling pathways such as Wnt/β-catenin, bone morphogenetic protein, and Notch. Particular emphasis is placed on circulating miRNAs as minimally invasive biomarkers, offering prospects for early detection, disease stratification, and monitoring therapeutic response. Furthermore, we discuss methodological advances in miRNA profiling, challenges in clinical translation, and the need for standardized validation frameworks. By integrating molecular insights with translational perspectives, this review underscores the promise of stem cell-associated miRNAs as diagnostic tools in osteogenic disorders, while identifying critical gaps that must be addressed to enable their incorporation into precision medicine strategies.

Mitochondrial quality control of mesenchymal stem cells in bone-related diseases: from mechanisms to emerging therapeutic opportunities

Bone-related diseases represent a significant global health challenge characterized by compromised bone mass and dysregulated skeletal metabolism. Mesenchymal stem cells (MSCs) are indispensable for maintaining bone homeostasis and facilitating tissue repair. However, their regenerative capacity is severely compromis...

Single-cell profiling reveals senescent bone marrow mesenchymal stem cell subpopulations driving pathological ossification in ankylosing spondylitis.

OBJECTIVES Aberrant new bone formation is a hallmark of ankylosing spondylitis (AS), yet the cellular mechanisms remain unclear. Bone marrow-derived mesenchymal stem cells (BMSCs) are crucial for skeletal homeostasis and may be pathologically reprogrammed under chronic inflammation. We aimed to characterize the cellula...

Mesenchymal stromal cells in age-related neurodegenerative disorders: mechanisms, translational barriers, and precision strategies

Ageing remodels the neuroimmune, metabolic, and vascular environment in ways that accelerate neuronal vulnerability and constrain tissue repair. Mesenchymal stromal cells (MSCs) and MSC-derived extracellular vesicles (MSC-EVs) are therefore attractive candidates for neurodegenerative disorders because their effects ext...

Exosome-mediated muscle-bone crosstalk: mechanisms and therapeutic potential

Muscle and bone maintain homeostasis through biomechanical and molecular crosstalk. Disruption of this interaction leads to osteosarcopenia, a condition marked by concurrent muscle and bone loss. Exosomes carry proteins and nucleic acids between tissues and are now recognized as key mediators of intercellular communica...
