Abstract
, Growing evidence on the use of endovascular thrombectomy (EVT) for medium and distal vessel occlusions (MDVOs) has sparked important debate about how mechanical reperfusion should be applied beyond the setting of large vessel occlusion (LVO) stroke. Recent observational and randomized data have raised concerns regarding the efficacy and safety of EVT in MDVO. In the large SITS International Stroke Register (SITS-ISTR) analysis by Keselman et al ., the addition of EVT to intravenous thrombolysis (IVT) was associated with lower rates of functional independence and higher mortality compared with IVT alone after propensity-score matching. [6] Together with randomized trials reporting neutral or negative results, these findings have intensified the debate regarding whether EVT is intrinsically limited in MDVO stroke or whether current outcomes reflect limitations in patient selection, technology, and procedural implementation. [2,3,9] However, interpretation of these findings requires caution. e SITS-ISTR analysis is an
Background Medium vessel occlusions (MeVOs) account for 25–40% of acute ischemic strokes and present a significant therapeutic frontier. Despite high technical success rates of endovascular thrombectomy (EVT), recent randomized trial results have been nuanced, creating a paradox and underscoring the need for a precisio...
Macrowire-assisted aspiration thrombectomy appears safe and feasible for MeVO, enabling efficient catheter delivery and high rates of successful reperfusion.

BACKGROUND
Mechanical thrombectomy has become the cornerstone of treatment for acute ischemic stroke (AIS) caused by large vessel occlusion. While randomized trials have established non-inferiority of aspiration-based approaches compared with stent retriever approaches, real-world evidence is essential to evaluate devi...

The clinical and procedural benefits of balloon guide catheter (BGC) use during mechanical thrombectomy remain debated, and whether these associations vary by occlusion location is uncertain. We evaluated the association of BGC use with thrombectomy outcomes and examined potential effect modification by occlusion loc...

Current evidence does not support routine EVT in unselected DMVO, and EVT was not associated with improved functional or survival outcomes in unselected patients with DMVO.
