Abstract

Some patients with HER2-positive metastatic breast cancer (MBC) achieve prolonged disease control with first-line anti-HER2-based therapy, but associated clinical features remain poorly characterized in real-world populations. We conducted a retrospective study using the Epidemiological Strategy and Medical Economics (ESME) MBC cohort, including 18 French comprehensive cancer centers. Women with HER2-positive MBC who initiated first-line anti-HER2-based therapy between January 2008 and December 2021 and had at least 3 years of follow-up were eligible. Exceptional responders had no documented progression within 3 years. A secondary analysis evaluated factors associated with remaining progression-free at 5 years. Multivariable logistic regression with bootstrap cross-validation was used. Among 4307 eligible patients, 1016 (23.6%) were exceptional responders. Of 765 evaluable at 5 years, 523 (68.4%) remained progression-free. Factors associated with a lower likelihood of exceptional response included hormone receptor-negative (HR-) disease, a disease-free interval of 6-24 months, ≥3 metastatic sites, visceral metastases, and central nervous system involvement. First-line treatment with chemotherapy, trastuzumab, and pertuzumab was associated with exceptional response. Among exceptional responders, HR-disease, grade III tumors and first-line chemotherapy plus trastuzumab and pertuzumab were associated with progression-free at 5-year, whereas visceral metastases were associated with lower odds. Model discrimination was moderate (AUC 0.72 and 0.64, respectively). Nearly one quarter of patients remained progression-free for at least 3 years, and more than two thirds of those evaluable remained progression-free at 5 years. Clinical variables alone had limited predictive value, supporting the need for molecular and immune biomarkers to better identify this subgroup.

Efficacy of HER2-targeted therapy in early breast cancer 2008–2020 in Sweden in the real-world setting

Large-scale real-world evidence on outcomes in HER2-positive (HER2 +) early breast cancer (EBC) remains limited. We investigated overall survival (OS) and its prognostic factors in patients with HER2+ EBC. We here analyzed data from 4,638 patients with HER2 + EBC diagnosed between 2008 and 2020, retrieved from the Swed...

E7389-LF as a first-line chemotherapy for patients with metastatic/advanced HER2-negative breast cancer: results from the phase 1 dose-expansion part of study 114

The results highlight the antitumor activity and manageable safety profile of E7389-LF, indicating the potential of E7389-LF as a 1L treatment for patients with HER2-negative breast cancer, warranting further study.

Real-world treatment patterns and associated outcomes in patients with HER2-positive unresectable or metastatic breast cancer: the HER2 REAL Study

Background The HER2 REAL (NCT04857619) retrospective study explored treatment practices and survival outcomes in patients with human epidermal growth factor receptor 2 (HER2)–positive unresectable breast cancer/metastatic breast cancer (mBC) in routine clinical care across six countries. Methods Adult patients (≥18 yea...

First-line treatment outcomes in HER2-altered advanced NSCLC: a real-world multi-center study comparing trastuzumab deruxtecan with conventional therapies and analysis by HER2/EGFR co-alteration status

First-line treatment options for HER2-altered advanced non-small cell lung cancer (NSCLC) remain limited, especially in cases with HER2-EGFR co-alterations. While new agents like trastuzumab deruxtecan (T-DXd) have emerged, its real-world evidence in first line remains limited. This study included patients with advance...

Real-World First-Line Treatment Characteristics and Physician Drivers in Advanced/Metastatic Non-Small Cell Lung Cancer, PD-L1 ≥ 50%, and without Actionable Genomic Alterations.

INTRODUCTION For patients with advanced/metastatic non-small cell lung cancer (aNSCLC) without actionable genomic alterations (AGAs) and with programmed death-ligand 1 (PD-L1) ≥ 50%, first-line (1L) treatment options include immunotherapy (IO) monotherapy or IO with chemotherapy. However, real-world data describing tre...
