Temporal lobectomy for glioma surgery: how I do it
Abstract
Anterior temporal lobectomy is well standardized in epilepsy surgery, but its oncological application is less defined. Tumor-induced distortion of anatomical landmarks, proximity of eloquent cortex and white-matter tracts, and the need to balance maximal resection with functional preservation demand a reproducible surgical framework. We describe a four-step technique for oncological temporal lobectomy defined by four anatomical limits: posterior, superior, medial, and the temporal stem. A standardized four-step approach to oncological temporal lobectomy allows reproducible execution of a supramaximal resection while preserving function. The technique is detailed in an accompanying video.
