TL;DR

SGLT2 inhibitor use was associated with favorable cardiovascular outcomes in patients with moderate-to-severe psoriasis receiving biologic therapy, and these findings support further prospective investigation of SGLT2 inhibitors as a potential strategy for cardiovascular risk reduction in this population.

Abstract

Background

Patients with moderate-to-severe psoriasis have an increased risk of cardiovascular disease. Sodium-glucose cotransporter 2 (SGLT2) inhibitors provide cardiovascular benefits, but their association with cardiovascular outcomes in patients with psoriasis remains insufficiently studied.

Methods

We conducted a retrospective cohort study using the global TriNetX research network. Patients older than 40 years with plaque psoriasis receiving biologic therapy were categorized according to SGLT2 inhibitor use. Propensity score matching was performed to balance demographic characteristics, cardiovascular risk factors, kidney disease, and concomitant antidiabetic medications.

Results

After matching, 2,016 patients were included in each cohort. SGLT2 inhibitor use was associated with a significantly lower incidence of acute myocardial infarction (HR, 0.601; 95%CI, 0.426-0.849), stroke (HR, 0.553; 95%CI, 0.381-0.804), heart failure (HR, 0.751; 95%CI, 0.581-0.970), peripheral artery disease (HR, 0.648; 95%CI, 0.470-0.894), and angina pectoris (HR, 0.601; 95%CI, 0.404-0.894). No significant association was observed for transient ischemic attack.

Conclusions

SGLT2 inhibitor use was associated with favorable cardiovascular outcomes in patients with moderate-to-severe psoriasis receiving biologic therapy. These findings support further prospective investigation of SGLT2 inhibitors as a potential strategy for cardiovascular risk reduction in this population.

Association Between PCSK9 Inhibitor Use and Risk of Incident Heart Failure in Patients With Dyslipidemia.

AIM To evaluate whether the use of proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9is) is associated with a reduced 1-year incidence of new-onset heart failure (HF) compared to statins among patients with dyslipidemia. METHODS We conducted a retrospective cohort study using the TriNetX global health re...

SGLT2 Inhibitors and Cardiovascular Outcomes in Patients With Fabry Disease: Observations From a Federated Research Network.

BACKGROUND AND AIM Fabry disease (FD) is frequently complicated by renal impairment and cardiovascular events; however, evidence on sodium-glucose cotransporter-2 inhibitors (SGLT2i) in this population is scarce. Accordingly, we assessed the association between SGLT2i exposure and cardiovascular outcomes in adults with...

Comparative Cardiorenal Efficacy and Safety of SGLT2 Inhibitors and Finerenone in Chronic Kidney Disease With Type 2 Diabetes: A Systematic Review and Network Meta-Analysis.

AIMS No randomized trial has compared SGLT2 inhibitors with finerenone in chronic kidney disease (CKD) with Type 2 diabetes. We estimated their comparative cardiorenal efficacy and safety. MATERIALS AND METHODS MEDLINE, Embase, CENTRAL and trial registries were searched from inception to August 2026 for placebo-contr...

Preoperative SGLT2 Inhibitor Use and Outcomes After Major Cardiac Procedures: A Propensity Score-Matched Analysis of the TriNetX Network

Background/Objectives: Sodium–glucose cotransporter-2 (SGLT2) inhibitors improve cardiorenal outcomes in patients with diabetes, heart failure, and chronic kidney disease. Their association with perioperative outcomes after major cardiac procedures remains uncertain. Methods: In this retrospective cohort study using th...

SGLT2 Inhibitor Therapy and Risk of Incident Atrial Fibrillation in Patients With HFpEF and Type 2 Diabetes.

Background Atrial fibrillation (AF) frequently coexists with Heart Failure with Preserved Ejection Fraction, particularly in patients with Type 2 Diabetes, due to shared cardiometabolic and inflammatory mechanisms. Sodium-glucose cotransporter-2 inhibitors (SGLT2i) have demonstrated cardiovascular benefit in HFpEF, but...
