Abstract

Myasthenia gravis (MG) is a chronic autoimmune disease characterized by fluctuating skeletal muscle weakness resulting from impaired neuromuscular transmission, most commonly caused by autoantibodies against acetylcholine receptors (AChR). Immunosuppressive therapy remains the cornerstone of long-term disease management, particularly in patients with persistent or progressive symptoms despite symptomatic treatment. This literature review aims to summarize current evidence regarding the role of immunosuppressive therapy in MG, including indications, mechanisms of action, therapeutic efficacy, and safety profiles. The review was conducted using recent scientific literature, international consensus statements, and clinical practice guidelines. Current treatment options include glucocorticoids (prednisone and methylprednisolone) as first-line immunosuppressive agents, while non-steroidal immunosuppressants such as azathioprine, mycophenolate mofetil, tacrolimus, and rituximab serve as steroid-sparing alternatives or are indicated in selected clinical settings. Recent systematic review and network meta-analysis evidence further supports the effectiveness of immunosuppressive agents and monoclonal antibodies in improving clinical outcomes in patients with myasthenia gravis, highlighting the importance of individualized treatment selection based on disease characteristics and safety considerations. Overall, immunosuppressive therapy remains a cornerstone of MG management, and individualized therapeutic strategies combined with careful monitoring are essential to optimize disease control and improve patients' quality of life.
