Intestinal Ultrasound in the Diagnostic Assessment of Inflammatory Bowel Disease and Infectious enterocolitis: A Case-Based Narrative Review

Oct 2026 · Inflammatory Intestinal Diseases · 0 citations

TL;DR

This case-based narrative review describes how IUS can support diagnostic assessment in patients with suspected IBD activity or infectious enterocolitis, and highlights characteristic pattern-based IUS findings across common causes of infectious enterocolitis and intestinal tuberculosis.

Abstract

Background: Intestinal ultrasound (IUS) provides rapid, noninvasive assessment of transmural inflammation in inflammatory bowel disease (IBD). In clinical practice, infectious enterocolitis frequently mimics IBD activity or complicates established diseases, creating diagnostic uncertainty at presentation and increasing the risk of inappropriate treatment escalation. Summary: This case-based narrative review describes how IUS can support diagnostic assessment in patients with suspected IBD activity or infectious enterocolitis. We summarize core IUS parameters, including bowel wall thickness, mural vascularity, and bowel wall stratification, along with supportive findings, such as perienteric inflammatory fat, lymphadenopathy, and haustral changes. Validated IUS-based approaches for ulcerative colitis (UC) and Crohn’s disease (CD), including the Milan Ultrasound Criteria (MUC), International Bowel Ultrasound Segmental Activity Score (IBUS-SAS), and transperineal ultrasound (TPUS) for rectal assessment, are summarized. Using representative cases, we highlight characteristic pattern-based IUS findings across common causes of infectious enterocolitis and intestinal tuberculosis, focusing on disease distribution, symmetry, perienteric involvement, and short-interval evolution on follow-up examinations. These findings will help refine the differential diagnosis and inform subsequent testing in real-world clinical settings. Key Messages: IUS provides readily available, segment-based information that complements clinical evaluation when differentiating IBD activity from infectious enterocolitis. Pattern recognition on IUS may enhance diagnostic safety and guide subsequent investigations; however, findings should be interpreted in the context of clinical presentation and additional diagnostic modalities.

Intestinal Ultrasound for Inflammatory Bowel Disease: Expanding the Role of Point-of-Care Ultrasound in the Emergency Department.

Patients with inflammatory bowel disease (IBD) frequently utilize emergency department (ED) care. Intestinal ultrasound (IUS) is a noninvasive, point-of-care, radiation-free imaging modality increasingly used in outpatient IBD management. The purpose of this article is to introduce the use of IUS for the evaluation of...

Intestinal ultrasound assessment of Crohn’s disease activity and complications: correlation with clinical, biochemical, and cross-sectional imaging findings

Crohn’s disease (CD) is a chronic inflammatory bowel disorder with variable clinical presentation and disease course. Accurate, non-invasive assessment of disease activity is crucial for guiding management and follow-up. Intestinal ultrasound (IUS), particularly the International Bowel Ultrasound Segmental Activity Sco...

Diagnostic Value of Gastrointestinal Ultrasound (GIUS), Including Doppler and Contrast-Enhanced Ultrasound (CEUS), in the Assessment of Inflammatory Bowel Disease Activity: A Scoping Review

Background/Objectives: Treat-to-target management of inflammatory bowel disease (IBD) requires frequent, accurate assessment of disease activity. Endoscopy remains the reference standard, but its invasiveness limits repeated use, which has increased interest in gastrointestinal ultrasound (GIUS). To map the evidence on...

Transabdominal Intestinal Ultrasound for the Assessment of Ileal Disease Activity in Crohn's Disease.

Background and study aims Transabdominal intestinal ultrasonography (IUS) is increasingly used to assess transmural inflammatory activity in Crohn's disease (CD). This study evaluated the diagnostic performance of IUS for detecting ileal inflammatory activity in CD by comparing individual ultrasonographic parameters an...

Intestinal ultrasound: a non-invasive alternative to monitor and predict biologic treatment persistence in patients with inflammatory bowel disease.

BACKGROUND AND AIMS Patients with Inflammatory bowel disease (IBD) are at risk of disease progression. We examined the ability of transabdominal intestinal ultrasound (IUS) to predict treatment persistence to biologics in patients with IBD. METHODS One-year prospective observational cohort study. Clinical disease act...
