TL;DR

This is the first report of a patient with a double MODY variant resulting in early protein termination, and the available evidence suggests that all variants may be pathogenic.

Abstract

ABSTRACT Objectives We report a rare case of digenic MODY. In 2016, at age 15, the patient was diagnosed with type 1 diabetes and began insulin therapy. After discrepancies in her glycaemic control were observed, an extensive differential diagnosis was conducted. In 2024, genetic testing confirmed monogenic diabetes, allowing for tailored treatment of the patient. Methods and Results The initial diagnosis was type 1 diabetes, later revised to MODY after Sanger screening. Genetic testing detected c.293C>T, p.Ala98Val in the HNF1A gene, which was initially misinterpreted as a variant causing Maturity Onset Diabetes of the Young (MODY), an autosomal dominant monogenic diabetes. Eventually, next‐generation sequencing (NGS) revealed three heterozygous variants in the MODY genes: HNF1B and PDX1. The HNF1B and PDX1 genes are transcription factors responsible for pancreas development and insulin gene transcription. Two variants located very close to each other in the PDX1 gene were identified: c.694G>A, p.Gly232Ser and c.697G>T, p.Glu233Ter. The first causes an amino acid change, while the latter leads to an early termination codon. The third variant, c.827_828insT, p.Gly277ArgfsTer17, was found in the HNF1B gene, resulting in a premature STOP codon and a truncated nonsense protein product. This is the first report of a patient with a double MODY variant resulting in early protein termination. Overall, the available evidence suggests that all variants may be pathogenic. The variants' severity and likely digenic nature matched the proband's diabetes presentation, which didn't respond to oral agents and required insulin. Neither parent nor brother, each with a detected variant in a single gene, has developed diabetes. However, renal cysts were discovered in the mother and brother, and the father was diagnosed with prediabetes. Conclusions This case highlights the severe presentation of likely digenic MODY, similar to type 1 diabetes. As NGS use increases, more patients like these are expected to be identified.

A personalized approach to the treatment of neonatal diabetes mellitus caused by a KCNJ11 mutation: clinical observations

Neonatal diabetes mellitus (NDM), or diabetes of the newborn, belongs to the group of monogenic forms of the disease and is characterized by marked genetic polymorphism. It is a rare genetically determined disorder with early onset (typically within the first 6 months of life), manifesting as persistent hyperglycemia...

Case Report: A novel 1, 171 bp GCK promoter/exon 1 deletion in a Chinese pedigree: correcting a misdiagnosis of MODY10 based on an incidental INS variant

Background Maturity-onset diabetes of the young (MODY) comprises a heterogeneous group of autosomal dominant monogenic diabetes disorders; accurate molecular diagnosis is essential, as different subtypes require fundamentally distinct management strategies. Objective In a Chinese pedigree initially misdiagnosed as MODY...

Compound heterozygous and digenic inheritance in early-onset Parkinson’s disease: a systematic review

Compound heterozygous (CH) genotypes represent an established biallelic autosomal-recessive mechanism in early-onset Parkinson’s disease (EOPD), whereas digenic inheritance (DI) has been proposed for variant combinations across two genes but is more difficult to establish. We systematically evaluated the genetic ar...

A Heterozygous Variant in the GABBR2 Gene in a Girl With Clinical Classic Rett Syndrome

ABSTRACT Background Rett syndrome (RTT) is a neurodevelopmental disorder mainly affecting females and may start with seemingly normal early development but leads to developmental stagnation, regression, and characteristic neurological symptoms. While most cases involve MECP2 variants, other genes have been implicated i...

Early-onset parkinsonism as a presenting feature of suspected type 1 Gaucher disease with two pathogenic GBA1 variants: a case report

Parkinson's disease (PD) is a common neurodegenerative disorder, and pathogenic variants in GBA1 are among its most frequent genetic risk factors. Biallelic pathogenic GBA1 variants cause Gaucher Disease (GD). In patients with parkinsonism and two detected GBA1 variants, the absence of biochemical confirmation and alle...
