Abstract
Pulmonary arterial hypertension is a progressive and serious disease characterized by pulmonary vascular remodeling and subsequent right ventricular failure. In Argentina, the management of this condition faces significant health challenges, evidenced by diagnostic delays, under-utilization of recommended therapies, and difficulties in accessing and maintaining long-term treatment. The current medical paradigm promotes a dynamic, multiparametric approach focused on risk stratification to achieve low long-term risk. Pharmacological treatment is usually based on three classic pathways (nitric oxide, endothelin, and prostacyclin), recently complemented by innovative disease-modifying agents such as sotatercept. However, the complexity of management, the high cost of specific drugs, and difficulties in the regular provision of medication lead to poor adherence and equitable access, making the development of public health policies adapted to the local context an urgent issue.
