A hybrid approach to full-mouth rehabilitation with telescopic overdentures on zirconia primary copings: a case report
Abstract
Background. Rehabilitation of a severely compromised dentition is challenging when few strategic teeth remain and implant-supported treatment is limited by ridge resorption, oral hygiene limitations, or cost. Tooth-supported telescopic overdentures may provide a maintainable alternative in selected patients. Case report. A 45-year-old female patient presented with failing fixed prostheses, chronic periapical lesions, periodontal disease, esthetic impairment, and masticatory dysfunction. Treatment included staged extraction of hopeless teeth, endodontic and restorative stabilization of maintainable abutments, and prolonged provisionalization. After the healing phase, residual ridge resorption, lip support, and restorative space requirements led to a revised plan consisting of maxillary and mandibular telescopic overdentures. Zirconia primary copings, milled titanium secondary frameworks, TK-Soft friction elements, intraoral scanning, conventional elastomeric impressions, and facebow transfer were combined in a hybrid workflow. Conclusion. At the 3-month follow-up, the prostheses remained clinically functional after minor adjustments. Longer follow-up is needed to assess retention, maintenance requirements, and abutment prognosis.
