Available evidence suggests that this approach is associated with faster postoperative recovery and a reduced risk of postoperative dislocation, supporting the MIS-AL approach as a reliable and muscle-sparing option for primary THA.
Abstract
The anterolateral approach has been widely adopted in total hip arthroplasty (THA), with numerous modifications described since its original introduction. Among these, the minimally invasive anterolateral (MIS-AL) approach, also referred to as the anterior-based muscle-sparing approach, has been developed to minimize soft-tissue damage by preserving the integrity of the gluteus medius muscle. This narrative review describes the surgical technique of the MIS-AL approach for primary THA performed in the supine position and provides a summary of clinical outcomes. When performed in the supine position, the MIS-AL approach offers several advantages beyond its muscle-sparing nature. It does not require a specialized operating table, allows for a simplified operating room setup, facilitates the use of intraoperative fluoroscopy, and enables simultaneous bilateral THA without the need for re-draping. In addition, available evidence suggests that this approach is associated with faster postoperative recovery and a reduced risk of postoperative dislocation, supporting the MIS-AL approach as a reliable and muscle-sparing option for primary THA.
The direct anterior approach (DAA) has gained widespread acceptance for primary total hip arthroplasty because it utilizes an intermuscular and internervous interval, minimizes soft tissue disruption, and facilitates rapid postoperative recovery. Although its application in primary arthroplasty is well established, fem...

Background: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate p...

Abstract Introduction: The long-term success of total hip arthroplasty (THA) requires optimal acetabular component positioning. To improve implant placement, intra-operative fluoroscopy is increasingly used, yet the evidence remains limited in the Indian population and confined to the direct anterior approach. This stu...

Current comparative evidence does not demonstrate a consistent long-term functional or major-complication advantage over contemporary PA/PLA, and surgical approach should therefore be individualized according to patient characteristics, surgeon experience, and institutional practice.
