Orthognathic surgery with clear aligners: methods of intermaxillary fixation in transsurgical phase

Sep 2026 · REVISTA DELOS · 0 citations

Abstract

The treatment of dental malocclusion is generally performed through orthodontic therapy. However, in cases where there are hereditary or acquired skeletal alterations, orthognathic surgery may be necessary. This study aims to discuss the advantages and disadvantages of using clear aligners compared to conventional orthodontic appliances in preparation for orthognathic surgery. Materials and Methods: A search was conducted in the PubMed/Medline database using the terms "Orthognathic Surgical Procedures" AND "Invisalign". Fifty-six relevant references were identified, of which 13 studies were selected based on inclusion and exclusion criteria, including case reports, narrative reviews, and comparative studies. Results: Of the 13 selected studies, there were eight case reports, two narrative reviews, one comparative study, one preliminary study, and one retrospective case-control study. The studies analyzed various clinical characteristics, including the type of dentofacial deformity and the intermaxillary fixation methods used. Discussion: The studies showed that clear aligners offer aesthetic advantages and comfort during pre- and post-surgical orthodontic treatment. However, the effectiveness of aligners in immediate postoperative control and suitability for complex cases of skeletal deformity still raises concerns among professionals and patients. Moreover, the choice between aligners and conventional appliances may depend on factors such as the severity of the deformity and patient preferences. Conclusion: Clear aligner therapy is a viable alternative to conventional orthodontic appliances in preparation for orthognathic surgery, offering significant aesthetic benefits. However, the decision should be based on a careful evaluation of the patient's individual needs and the specific characteristics of the case.
