A 65-year-old woman with stable fibrotic ILD associated with Sjögren–antisynthetase overlap syndrome who developed severe precapillary PH is reported, supporting further evaluation of the efficacy and safety profile of sotatercept in PH with concomitant ILD.

Abstract

Sotatercept is a drug approved for pulmonary arterial hypertension (PAH) that improves mortality, right ventricular function, functional class, NT-proBNP, and 6-minute walk distance. However, no clinical trials support its use in patients with pulmonary hypertension (PH) and interstitial lung disease (ILD), and respiratory adverse effects have been reported due to the development of intrapulmonary shunts. We report a 65-year-old woman with stable fibrotic ILD associated with Sjögren–antisynthetase overlap syndrome who developed severe precapillary PH. Despite treatment with inhaled treprostinil and tadalafil, she showed no clinical or hemodynamic improvement. Sotatercept and macitentan were initiated to improve hemodynamics and serve as a bridge to transplantation. After three months, the patient achieved functional class I, BNP reduction, normalization of echocardiographic PH findings, and marked hemodynamic improvement. Despite concerns regarding sotatercept-associated intrapulmonary shunts and hypoxemia, basal oxygen saturation improved from 93% to 96%. Bilateral lung transplantation was performed without immediate complications. This case supports further evaluation of the efficacy and safety profile of sotatercept in PH with concomitant ILD.

Case Report: Upfront sotatercept-based triple therapy in high-risk systemic sclerosis-associated pulmonary arterial hypertension

Systemic sclerosis-associated pulmonary arterial hypertension (SSc-PAH) carries a poor prognosis, and treatment decisions are often complicated by coexisting left-heart disease, lung disease, chronic thromboembolic disease, or pulmonary veno-occlusive disease-like vasculopathy. The role of sotatercept as part of initia...

Sotatercept in Pediatric Pulmonary Hypertension: Experience from the Pediatric Pulmonary Hypertension Network.

BACKGROUND Sotatercept, a ligand trap for TGF-β family members, has been shown to improve exercise capacity and decrease pulmonary vascular resistance (PVR) in adults with pulmonary arterial hypertension. Limited data exist describing the use of sotatercept in children with pulmonary hypertension (PH). RESEARCH QUEST...

Pulmonary hypertension in interstitial lung disease: today's realities, tomorrow's possibilities.

Interstitial lung disease-associated pulmonary hypertension (ILD-PH) is a frequent and life-limiting complication of fibrotic lung disease, associated with substantial morbidity, impaired functional capacity, and significantly reduced survival. Under-recognition remains common due to overlapping symptoms with ILD, evol...

Investigation of the Effects of Switching from Bosentan to Macitentan in Pediatric Pulmonary Hypertension Patients

Objectives Pulmonary arterial hypertension (PAH) is a progressive disease characterized by increased pulmonary vascular resistance and reduced exercise capacity. Bosentan, an endothelin receptor antagonist, is commonly used in pediatric PAH, but standardized guidelines for its use are lacking. Macitentan, another endot...

Management of Pulmonary Hypertension in Interstitial Lung Disease Without Access to Inhaled Treprostinil: A Case Series from a Resource-Limited Setting.

BACKGROUND Pulmonary hypertension (PH) may develop as a clinically important complication of parenchymal lung diseases, particularly in the setting of chronic hypoxemia and advanced interstitial lung disease. The management of interstitial lung disease-associated pulmonary hypertension (ILD-PH) remains challenging and...
