Risk Analysis of Handy Talky Falling into a Cargo Tank During the Loading Process on the Operational Safety of Tanker Vessels

Abstract

This study analyzes an incident in which a Handy Talky (HT) fell into the portside No. 3 cargo tank during aviation fuel loading at Jetty Pertamina RU VI Balongan on September 19, 2024, posing potential operational disruptions and significant safety risks such as fire and explosion. The research was conducted onboard MT MPMT XV using a qualitative approach, with data collected through observation, discussions with the crew, and document review. Data validity was ensured through technique and source triangulation, while analysis was carried out using a fishbone diagram to identify the causes and USG (Urgency, Seriousness, Growth) analysis to determine priority actions. The findings indicate that the main cause of the incident was crew negligence in not using a safety lanyard when carrying the handy talky. The incident created serious safety hazards that could threaten crew safety, cargo security, and operational continuity. Mitigation actions were implemented by prioritizing cargo shifting followed by tank cleaning to retrieve the device. As a preventive measure, the mandatory use of safety lanyards on all handy talky devices onboard is recommended to reduce the risk of similar incidents in the future.
