Vaccination practice and safety in children with special health care needs: A retrospective study of 1278 children

Abstract

ABSTRACT Children with special health care needs (CSHCN) are at increased risk of vaccine-preventable diseases, yet concerns about safety often lead to delayed or incomplete immunization. We conducted a retrospective analysis of 1278 CSHCN attending the Special Vaccination Consultation Clinic of Kunming Children’s Hospital in Yunnan Province, China, between January 2023 to December 2024. We examined their disease spectrum, vaccination recommendations, and incidence of adverse events following immunization (AEFI). The cohort had a male-to-female ratio of 1.3:1, with infants aged 0–1 y constituting the majority (56.4%). Neurological (38.3%), neonatal (33.6%), and cardiovascular diseases (21.4%) predominated the consultation spectrum, with a marked age-dependent shift from neonatal conditions in early infancy to neurological disorders thereafter. Following specialist evaluation, 92.5% of children received vaccination advice: 76.7% were recommended for standard or single-dose sequential vaccination, and 15.8% for inactivated vaccines only; only 2.7% were advised to defer all vaccinations. Among 1182 children who were subsequently vaccinated, 174 reported AEFI, yielding an incidence of 14.7%. Fever was the predominant manifestation (70.7% of AEFI), and the vast majority (99.4%) of events occurred in children under 4 y of age. This study demonstrates that under the guidance of a specialized vaccination clinic, over 90% of CSHCN can be safely vaccinated. Individualized assessment based on age and disease characteristics is pivotal to ensuring immunization in this vulnerable population, thereby safeguarding their right to protection against vaccine-preventable diseases.

Assessment of child and maternal immunization coverage among patients at medical college hospital: A hospital-based cross-sectional study

Background: Immunization is thought to be a very successful and economical therapeutic strategy since it dramatically lowers death and disability from vaccine-preventable illnesses. The survey was carried out at Rangpur medical college hospital (RMCH), Rangpur Division, Bangladesh, with the aim of evaluating the vaccination makeup of kids and moms. Methods: During January and June 2025, systematic interviews and immunisation document checking served to gather information about 200 participants, comprising 120 mothers and 80 children. For the statistical assessment, both deductive and informative techniques were applied using SPSS version 25. Findings: According to the Expanded Programme on Immunisation (EPI) timeline, 65% of the children assessed (n = 80) did totally immunised, 25% did partly immunized, and 10% did none. 55% of mothers received tetanus toxoid (TT) boosters, 78% obtained TT2, and 90% obtained TT1. Mothers in the 20–30 age range showed the highest percentage of vaccination knowledge (65%), followed by those over 30 (55%) and under 20 (40%). Ignorance (42%), vaccine unavailability (28%), fear of complications (18%), and hospital proximity (12%) were the largest obstacles to complete immunization. Furthermore, compared to children from distant regions (45%), children from urban or semi-urban areas had higher full vaccination rates (72%) (p = 0.012). Conclusion: The results underscore the critical role of tailored community-based interventions and policy measures that address structural inequities, maternal empowerment, and healthcare accessibility. To increase equitable levels of vaccination throughout Bangladesh, the results emphasize the necessity of enhancing maternal welfare skills, ensuring that vaccines remain available, and integrating immunizations into routine healthcare for women and newborns. Novelty/Originality of this article: The study additionally explored the interplay of socioeconomic status, maternal education, and health system factors, providing a holistic understanding of immunization behavior and the underlying social determinants influencing vaccine uptake.

Parental preferences for immunization advisory clinic settings for children with special health care needs and associated factors in Shanghai: A cross-sectional study

ABSTRACT Children with special health care needs (CSHCN) face persistent immunization inequity despite high overall childhood vaccination coverage. Immunization advisory clinics (IACs) have expanded in China, yet parental preferences for different IAC settings remain poorly understood. This cross-sectional survey was conducted at the IAC of Children’s Hospital of Fudan University, Shanghai, between October and December 2023. Parents of CSHCN younger than 14 y with incomplete National Immunization Program schedules were invited. The primary outcome was parental preference for IAC setting (community clinic, general hospital, or children’s hospital); the secondary outcome was preferred vaccination-assessment provider under discordant recommendations. Multinomial and binary logistic regression models identified associated factors, adjusted for child, parent, and vaccine-attitude covariates. Sensitivity analyses were stratified by medication status. Reporting followed the STROBE statement. Of 102 questionnaires, 101 were valid (99.0% response rate). 51.0% of parents preferred community clinics, 25.7% preferred children’s hospitals, and 23.3% preferred general hospitals. Parent Attitudes about Childhood Vaccines (PACV) score was not independently associated with setting preference. Parents of girls were more likely to prefer general hospitals (adjusted RRR, 7.02; 95% CI, 1.50–32.77), whereas lower household income was associated with reduced preference for general hospitals (adjusted RRR, 0.14; 95% CI, 0.03–0.75). Sensitivity analyses yielded consistent findings. Parental preferences for IAC settings among CSHCN families are shaped by child and socioeconomic characteristics rather than by vaccine hesitancy alone. Hybrid hospital-community delivery models and standardized referral pathways may improve vaccination equity for medically complex children.

Healthcare professionals' perspectives and preferences regarding adolescent meningococcal disease vaccination in the United States.

BACKGROUND Invasive meningococcal disease (IMD) is a vaccine-preventable, serious disease. In 2023-2024, the United States (US) Advisory Committee on Immunization Practices (ACIP) proposed revisions to the adolescent meningococcal vaccine schedule. This survey aimed to understand healthcare professionals' (HCPs') perspectives on these revisions and the potential role of the meningococcal serogroups A, B, C, W, Y (MenABCWY) vaccine. METHODS An online survey was conducted in May - June 2025 among US HCPs who regularly recommend meningococcal vaccines for 16-23-year-old patients. Responses were analyzed descriptively. RESULTS Of 302 HCPs, 73.2% felt the MenABCWY vaccine provides significant value to the meningococcal vaccination program, and almost half preferred a two-dose MenABCWY schedule over the ACIP recommendation, wherein a MenABCWY dose is followed by a MenB dose. Most HCPs indicated concern with removing MenACWY vaccination for 11-12-year-olds (84.8%). Comparatively, fewer were concerned about changing the MenB vaccine recommendation from shared clinical decision-making to risk-based (60.6%) and administering the second MenB dose closer to college entry (48.7%). CONCLUSIONS While HCPs were receptive to revisions that simplify the vaccine schedule, this was not without concerns. The adolescent vaccination platform is essential to ensure continued protection against IMD; any changes will require clear, evidence-based guidance.
