Abstract

A BSTRACT Chronic left atrial (LA) pressure overload and rheumatic inflammation cause subclinical atrial remodeling even without severe valvular stenosis. This study intended to determine whether advanced atrial deformation parameters identify patients with increased risk of atrial fibrillation (AF) from mild rheumatic mitral stenosis (MS). A prospective cohort study of patients with mild rheumatic MS and sinus rhythm was conducted in Baghdad, Iraq. LA Reservoir Strain (LASr) and LA Mechanical Dispersion (LAMD) were calculated via two-dimensional strain echocardiography at baseline. Patients were followed up for at least 2 years. The primary outcome was the occurrence of AF. Of 211 patients, 39 were found to have new-onset AF. The two strain parameters remained independent predictors of new-onset AF on multivariable Cox proportional hazards regression analysis. New AF occurrence was positively correlated with LAMD (adjusted Hazard ratio [aHR]: 1.047 [95% confidence interval (CI): 1.02–1.06], P < 0.001) and inversely correlated with LASr (aHR: 0.848 [95% CI: 0.79–0.90], P < 0.001). The optimal thresholds for LAMD and LASr for predicting new AF were >58.7 ms and ≤16.6%, respectively, which were defined by the Receiver Operating Characteristic curve. The presence of subclinical electromechanical dysfunction is an independent predictor of AF in mild rheumatic MS. Recognizing this high-risk phenotype supports closer ambulatory rhythm monitoring to allow timely arrhythmia detection and subsequent initiation of anticoagulation to prevent thromboembolic events.

Prediction of left atrial appendage dysfunction by atrial strain imaging in patients with rheumatic mitral stenosis in sinus rhythm

Left atrial strain imaging provides a robust non-invasive predictor of LAA dysfunction in severe rheumatic MS patients in sinus rhythm and supports the potential clinical utility of LA strain in improving risk stratification and guiding anticoagulation decisions in this under-recognized high-risk population.

Left atrial dimension and new-onset atrial fibrillation in chronic Chagas disease: A competing-risk analysis of the SEARCH-Rio cohort.

In CCD, baseline LA diameter predicts new-onset AF over 25 years, with 4.0 cm acting as a dynamic threshold, and P-SAECG is unrelated to LA size and poorly predicts AF.

Right atrial functional remodeling for early risk stratification of tricuspid regurgitation progression in atrial fibrillation.

Impaired RA reservoir strain was independently associated with TR progression in AF patients with initially insignificant TR, suggesting that RA functional assessment may provide additional information complementary to RA structural remodeling and help identify patients at higher risk of TR progression.

Physiological Left Atrial Staging and the Risk of New-Onset Atrial Fibrillation in Hypertrophic Cardiomyopathy.

The proposed LA staging framework offers mechanistic insights into how the LA adapts to progressive diastolic dysfunction in HCM and is associated with a graded risk of NOAF and may help tailor AF surveillance strategies.

Association Between Left Atrial Enlargement and Atrial Fibrillation in Elderly Patients with Left Ventricular Diastolic Dysfunction

Background: Left ventricular diastolic dysfunction (LVDD) is associated with chronically elevated left ventricular filling pressures that promote left atrial (LA) remodeling and increase the risk of atrial fibrillation (AF). Because AF is frequently asymptomatic during its early stages, identifying echocardiographic ma...

Echocardiographic Left Atrial Volumetric-mechanical Coupling Index for Prediction of Atrial Fibrillation after STEMI.

BACKGROUND Atrial fibrillation (AF) commonly complicates ST-segment elevation myocardial infarction (STEMI). Conventional echocardiographic measures may incompletely capture left atrial remodeling. The left atrial volumetric-mechanical coupling index (LACI-vm), integrating left atrial volume index (LAVI) and mitral ann...
