Comparative Visual Outcomes of Toric and Nontoric Intraocular Lenses Featuring Combined Diffractive Extended Depth of Focus and Multifocal Designs

Abstract

Purpose To assess and compare visual quality and subjective outcomes of the toric and nontoric versions of a trifocal intraocular lens (IOL) combining a diffractive extended depth of focus (EDoF) profile with a diffractive multifocal pattern. Methods This prospective cohort study included patients bilaterally implanted with the TECNIS Synergy or TECNIS Synergy Toric II IOL. At 3 months postoperatively, monocular and binocular uncorrected distance visual acuity (UDVA), corrected visual acuity (VA) at 100%, 50%, and 10% contrast, and binocular defocus curves were evaluated. A five‐point Likert scale questionnaire assessed satisfaction at different distances, perception of halos, and willingness to choose the same lens again. Results A total of 114 eyes (52 nontoric and 62 toric) were analyzed. Monocular UDVA was 0.02 ± 0.07 logMAR (nontoric) and 0.08 ± 0.11 logMAR (toric) (p = 0.02), while binocular UDVA was −0.05 ± 0.06 logMAR and −0.02 ± 0.10 logMAR, respectively (p = 0.13). Defocus curves showed VA better than 0.1 logMAR from +1.0 to −2.5 D in both groups. No significant differences in contrast sensitivity were observed between groups at any level (p > 0.05). Over 80% of patients in each group reported being satisfied or very satisfied at all distances. Halos were described as not or only slightly bothersome by 42% of patients. A small percentage expressed a low likelihood of choosing the same lens again (4% nontoric, 9% toric). Conclusion Both IOL models provided similar and effective visual restoration from distance to near. Despite some reported visual disturbances, patient satisfaction and visual quality were high and comparable between the toric and nontoric lenses when appropriately aligned.

Visual performance of a non-diffractive extended depth of focus IOL in stable early-stage Keratoconus: prospective pilot study

To evaluate visual performance, contrast sensitivity (CS), optical quality, and subjective satisfaction of Vivity extended depth-of-focus (EDOF) premium intraocular lens (pIOL) in early keratoconus (KC). In this prospective, interventional, single-center clinical study, ten patients with mild bilateral KC and cataract received AcrySof IQ Vivity pIOL. Pre- and postoperative assessments included monocular and binocular uncorrected and distance-corrected visual acuities at 4 m (UDVA, DCVA), uncorrected and distance-corrected intermediate and near visual acuities at 66 cm (UI66VA, DCI66VA) and 40 cm (UNVA, DCNVA), defocus curve, CS, wavefront aberrometry (RMS, PSF, Strehl ratio), halometry, and subjective outcomes. At 3 months, mean ± SD monocular DCVA in the dominant eyes (primary endpoint) was 0.06 ± 0.13 logMAR. A statistically significant improvement was observed between preoperative and postoperative monocular and binocular UDVA, UNVA, DCNVA (p < 0.001), and DCVA (p = 0.03). Binocular DCI66VA and UI66VA were 0.10 ± 0.15 and 0.14 ± 0.16 logMAR, respectively. The defocus curve was ≤ 0.20 logMAR from + 0.50 D to – 2.00 D. CS peaked under scotopic conditions at 6 cpd, total ocular RMS improved from 2.15 ± 1.98 to 1.09 ± 0.77 (p = 0.09). NEI-RQL-42 scores indicated favourable patient-reported outcomes, particularly for glare, clarity of vision, and satisfaction with correction. In this small prospective pilot study, bilateral implantation of a non-diffractive EDOF IOL was associated with favourable short-term visual outcomes in highly selected patients with stable early keratoconus. These preliminary findings suggest that early stable KC may not represent an absolute contraindication to non-diffractive EDOF IOL implantation, but larger comparative studies with longer follow-up are required.

Clinical Performance of an Isofocal Toric Intraocular Lens with Double C-Loop Haptics: A Multicenter Retrospective Study

Purpose To evaluate clinical outcomes in eyes with pre-existing astigmatism undergoing cataract surgery or refractive lens exchange with the implantation of an isofocal optic-design toric intraocular lens (IOL) with double C-loop haptics. Method This multicenter retrospective study comprised 100 eyes (65 patients) implanted with the Isopure Serenity toric IOL (BVI, Inc.) conducted across four sites. Postoperative evaluation at between 2 and 3 months included manifest refraction, monocular uncorrected and distance corrected visual acuity (UDVA and CDVA), monocular uncorrected and distance corrected intermediate visual acuity (UIVA and DCIVA, at 80 and 66 cm), monocular uncorrected and distance corrected near visual acuity (UNVA and DCNVA, at 40 cm), binocular defocus curve and rotational stability. Results Manifest refraction spherical equivalent and refractive astigmatism were −0.24±0.41D and −0.36±0.36D, respectively. About 79% and 97% of eyes were within ±0.50D and ±1.00D, respectively, and 80% and 96% of eyes were ≤0.50D and ≤1.00D of refractive cylinder, respectively. At distance vision, the mean monocular CDVA was −0.02±0.06 logMAR and 92.93% of eyes showed a CDVA of ≥20/20. At intermediate vision, the mean monocular DCIVA values were 0.10±0.07 and 0.17±0.10 logMAR at 80 cm and 66 cm, respectively; and 96.92% and 88.33% of eyes showed a DCIVA of ≥20/32 at 80 cm and 66 cm, respectively. At near vision, the mean monocular DCNVA was 0.31±0.08 logMAR and 75.44% of eyes showed a DCNVA of ≥20/40. The binocular depth of focus range was approximately 1.50D. The mean absolute IOL rotation was 2.73±2.58 degrees with all eyes having a rotation of <10 degrees. Conclusions Eyes with pre-existing astigmatism implanted with the Isopure Serenity toric IOL showed good distance visual acuity and functional intermediate vision, with high refractive accuracy and good rotational stability following cataract and refractive lens exchange surgery.
