Patients with multiple DNA damage response gene alterations showed distinct immune profiles, higher rates of durable clinical benefit, and longer progression-free survival, and these findings suggest that genomic DNA damage response alterations may help identify patients who are more likely to benefit from personalized immunotherapy strategies.
Abstract
Simple Summary Immune checkpoint inhibitors have improved outcomes for patients with advanced non-small cell lung cancer, particularly those with high programmed death-ligand 1 expression. However, a proportion of patients derive limited benefit from these treatments, highlighting the need for additional biomarkers to guide therapeutic decisions. In this study, we investigated genomic alterations in the DNA damage response pathway and their association with immunotherapy outcomes in advanced non-small cell lung cancer. Patients with multiple DNA damage response gene alterations showed distinct immune profiles, higher rates of durable clinical benefit, and longer progression-free survival. They also appeared to derive greater benefit from combined chemotherapy and immunotherapy. These findings suggest that genomic DNA damage response alterations may help identify patients who are more likely to benefit from personalized immunotherapy strategies.
Immune checkpoint inhibitors (ICIs) have radically changed the treatment landscape of non-small cell lung cancer (NSCLC) and significantly improved patients' prognosis in many settings of the disease. Nevertheless, clinical outcomes remain highly heterogeneous, and a substantial proportion of patients do not achieve me...

Simple Summary Circulating tumor DNA (ctDNA) can prognosticate survival outcomes in melanoma following checkpoint inhibition, but its role in advanced melanoma treated with radiation remains unclear. In this retrospective analysis of 50 patients with advanced-stage melanoma treated with checkpoint inhibition and pallia...

Lung cancer management remains constrained by a fragmented understanding of the clinical implications of DNA Damage Response (DDR) pathways. This review synthesizes current evidence to position DDR alterations as potential prognostic, predictive, and therapeutic biomarkers. Prognostically, overexpression of FEN1, BRCA1...

By combining precise tumor targeting with immune system activation, radiation and immunotherapy have emerged as powerful approaches to extend cancer patient survival and enhance quality of life. While much of this progress has focused on T cell-mediated responses, our study focuses on the role of B cells in predicting...
