TL;DR
This chapter provides a comprehensive synthesis of microglial biology across multiple interfaces, including transcriptomic, lipid-associated metabolic, cell senescence, and metabolic reprogramming, and positions microglia biology as a metabolically plastic, spatially organized immune-regulated microenvironment that can efficiently aid in reprogramming and AD therapeutics.
This review systematically outlines the fundamental characteristics of microglial glucose metabolism and focuses on how the dynamic metabolic reprogramming it undergoes during AD progression regulates microglial immune behavior and inflammatory responses.

It is shown that a disease-associated microglial subtype, characterized by elevated GPNMB expression and enriched for polygenic AD risk, expands with AD pathology and shows increased phagocytic activity, and MITF is identified as an upstream regulator required to maintain this microglial state.

This chapter focuses on the diversification of neurodegenerative CNS microglial states, the interaction of microglia with neurons, glial cells, adaptive immune cells, and the vasculature, and how this framework provides a basis for therapeutic strategies aimed at modulating, replacing, or reprogramming microglia in neu...

A “cellular state–pathological network–therapeutic window” framework is proposed and the roles of microglia in amyloid-β plaque seeding and compaction, NLRP3 inflammasome activation, mitochondrial DNA–cGAS–STING signaling, complement-mediated synaptic engulfment, and bidirectional microglia–tau feedback are systematica...

A distinct microglial state is identified that provides new insights into the dynamic role of microglia in AD and is associated with the phagocytosis of dystrophic neurites and was recapitulated in vitro using an efferocytosis-based model.

This work situates MiET formation within the broader microglial functional repertoire and focuses on how ET-dominant responses may compromise homeostatic or neuroprotective roles, expanding the conceptual framework for microglial pathogenicity and inviting future mechanistic and translational research.
