Motivators and barriers to influenza vaccination among healthcare workers

Abstract

ABSTRACT Aimed to identify the key motivators and barriers influencing influenza vaccination uptake among healthcare workers -and to examine the behavioral, social, and practical factors associated with vaccination acceptance and future vaccination intentions. A cross-sectional survey was conducted to assess the social and behavioral drivers (BeSD) of influenza vaccination among healthcare workers. A total of 523 Jordanian healthcare workers employed in healthcare institutions across the three regions of Jordan were recruited. Descriptive statistics, chi-square tests and Univariable logistic regression analyses were used. A total of 523 healthcare workers participated in the study, with 48.0% reporting receipt of the influenza vaccine within the previous three years. Awareness of influenza vaccination was high (96.2%), although only 24.1% considered its benefits very important and 31.7% perceived it as very safe. Previous vaccination experience was the strongest predictor of motivation to vaccinate (OR = 24.38, 95% CI: 15.33–38.76). Confidence in vaccine benefits (OR = 16.97, 95% CI: 10.95–26.29), vaccine effectiveness (OR = 15.08, 95% CI: 9.70–23.42), social responsibility (OR = 11.33, 95% CI: 7.51–17.09), and satisfaction with vaccination services (OR = 6.60, 95% CI: 4.45–9.79) were significantly associated with motivation to receive influenza vaccination (all p < .001). Influenza vaccination uptake among healthcare workers in Jordan remains suboptimal despite high awareness. Belief in vaccine benefits, effectiveness, and safety, along with previous vaccination experience, social influences, and vaccine accessibility, were key determinants of uptake and motivation. Targeted interventions addressing misconceptions and strengthening vaccine confidence may improve vaccination coverage among healthcare workers.

Current issues in the vaccination of healthcare workers.

OBJECTIVE Aim: To assess healthcare professionals' attitudes toward vaccination, evaluate vaccination coverage, and perform a SWOT analysis of immunoprophylaxis to identify strengths, weaknesses, opportunities, and threats. PATIENTS AND METHODS Materials and Methods: A structured, anonymous 23-item questionnaire assessed knowledge and attitudes toward immunoprophylaxis among healthcare workers (HWs), covering demographics, vaccination awareness, infection prevention practices, attitudes, and professional motivation. The online survey (Google Forms) was conducted from May to December 2025 with ensured anonymity. Vaccination coverage was assessed using records of 1060 HWs from two public healthcare facilities, with comparative analysis across departments. A SWOT analysis was performed. For relative indicators, 95% confidence intervals (CIs) were calculated, accounting for sampling error associated with sample size (MedStat v.5.2). RESULTS Results: The majority of study participants demonstrated a positive attitude towards immunoprophylaxis: 72.4% (CI: 63.8-81.0) rated it as absolutely positive. However, gaps were identified: correct understanding of current COVID-19 vaccination recommendations was 28% (CI: 19.4-36.6), while 74% (CI: 65.6-82.4) correctly assessed continuation of vaccination schedules. The SWOT analysis showed generally positive attitudes toward vaccination and potential for improving coverage through education and management interventions. CONCLUSION Conclusions: Despite positive attitudes (72.4%) and satisfactory self-assessed knowledge, vaccination coverage among healthcare workers remains low (<30%): 29.7% for tetanus-diphtheria and 27.5% for hepatitis B. This reflects a clear gap between knowledge, motivation, and practice, indicating the need for integrated interventions, including education, communication skills development, and strengthened institutional vaccination policies.

Factors affecting parental practices and attitudes toward influenza vaccination for children in China.

Investigation of parents' knowledge, attitudes, and practices regarding childhood influenza vaccination in Jiangsu showed moderate knowledge and positive attitudes but low actual vaccination rates, suggesting targeted interventions are needed to reduce the cognition-practice gap and improve childhood influenza vaccination coverage.

Factors associated with influenza vaccine uptake among healthcare workers in Georgia, 2021–2024

Background Healthcare workers (HCWs) are prioritised to receive influenza vaccine annually in Georgia. We aimed to identify factors associated with influenza vaccine uptake among Georgian HCWs. Methods We used self-reported influenza vaccination data for three influenza seasons (2021–2024) and 2023 survey data on attitudes towards influenza from a prospective cohort of HCWs in six hospitals in Georgia. HCWs who received influenza vaccine in at least one season were compared with those who were not vaccinated in any season. Adjusted odds ratios (aORs) and 95% confidence intervals (CIs) were calculated using multivariable logistic regression to identify factors associated with vaccination uptake. Findings Of 403 participants, the median age was 46 (IQR 36–54), 364 (90%) were female, 168 (42%) were nurses or midwives, 85 (21%) were support staff, 75 (19%) were doctors, and 75 (19%) were other staff. Overall, 289 (72%) received influenza vaccine in at least one season, while 114 (28%) did not receive it in any season. Multivariable analysis showed no differences in vaccine uptake by sex or occupation. HCWs ≥ 30 years were more likely to be vaccinated, with those aged 40–49 having the highest odds (aOR 2.67, 95% CI 1.09–6.59). HCWs who did not believe the vaccine was safe (aOR 0.27, 95% CI 0.11–0.68), those who doubted its effectiveness (aOR 0.36, 95% CI 0.14–0.89), and HCWs at Hospital B (aOR 0.06, 95% CI 0.02–0.23) were less likely to be vaccinated. Conclusions Georgian HCWs who questioned influenza vaccine safety and effectiveness, and those at one hospital, had lower odds of vaccination over three years, whereas older HCWs were more likely to be vaccinated. Future campaigns to promote influenza vaccination among HCWs should address safety and effectiveness concerns and target demographic disparities.
