Abstract
Background
Therapeutic options for inflammatory bowel disease (IBD) have expanded rapidly with the introduction of selective interleukin (IL)-23 inhibitors, Janus kinase inhibitors, sphingosine-1-phosphate receptor modulators, and several emerging therapeutic classes. Consequently, the major clinical challenge has shifted from developing effective therapies to determining how best to position them within increasingly complex treatment algorithms.
Aims
To review the contemporary and emerging therapeutic landscape in IBD, focusing on mechanism-based treatment positioning, disease modification, refractory disease, and novel therapeutic strategies that may overcome the current therapeutic ceiling.
Methods
We performed a comprehensive review of PubMed, Ovid MEDLINE, Embase, and major international conference proceedings to identify pivotal clinical trials, long-term extension studies, comparative effectiveness studies, real-world evidence, and emerging therapeutic developments in IBD.
Results
Comparative studies have strengthened the positioning of selective iL-23 inhibitors as foundational therapies in IBD, while oral small molecules continue to expand with novel mechanisms and improved convenience. Emerging strategies targeting TL1A, fibrosis and advanced combination therapy offer the potential to further improve outcomes, particularly in refractory disease. However, important challenges remain regarding optimal treatment sequencing, patient selection, long-term safety and disease modification.
Conclusion
Contemporary IBD management is increasingly defined by mechanism-based therapeutic positioning rather than simply expanding treatment options. Ongoing advances in comparative effectiveness research, novel therapeutic targets, and treatment strategies have the potential to further individualise care and improve long-term patient outcomes.
Background: Inflammatory bowel disease (IBD) is a chronic, relapsing disorder resulting from genetic, immune, microbial, and environmental factors. Therapeutic options have expanded from conventional drugs to biologic agents and targeted small molecules, enabling individualized management.

INTRODUCTION
Biologic therapies have transformed the management of severe asthma, particularly in type 2-high disease, but important unmet needs remain. Incomplete responses, uncertainty in treatment positioning, and the lack of effective options for non-type 2 or mixed inflammatory endotypes continue to limit the achi...

ABSTRACT Introduction Still’s disease is a systemic inflammatory disorder characterized by a complex cytokine network involving interleukin (IL)-1, IL‑6, IL‑18 and interferon (IFN)-γ. Although IL‑1 and IL‑6 inhibitors represent the cornerstone of current targeted therapy, a subset of patients remains refractory or deve...

Inflammatory bowel disease (IBD), comprising ulcerative colitis (UC) and Crohn’s disease (CD), is a chronic immune-mediated disorder of rising global prevalence, with newly industrialised nations now entering the phase of most rapid growth. Its therapeutic landscape has transformed since 2020. Three selective interleuk...
Introduction: Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by recurrent inflammation of the colon resulting from immune dysregulation. Despite the availability of multiple therapeutic options, a substantial proportion of patients fail to achieve sustained remission or develop loss of re...
