Bridging Gene-Editing Deficiencies in Solid Tumours: Cavitation-Gated Delivery, Spatial Mutation Mapping, and an Immune-Primed Multi-Stage Framework
Abstract
Focused-ultrasound histotripsy has established a non-thermal, non-ionising mechanical interface to living tissue: microsecond, high-amplitude negative-pressure pulses nucleate cavitation bubble clouds that fractionate cells while largely sparing collagenous matrix. This paper asks whether that interface can be redirected from ablation toward the correction of the genetic and non-genetic lesions that sustain solid tumours, and it maps with equal care where that ambition currently fails. We first fix the classical foundation (Keller-Miksis bubble dynamics, intrinsic threshold near 25-30 MPa, duty-cycle-limited heating) and then close off the quantum and informational routes that the dialogue initially explored: at 310 K decoherence destroys any coherent acoustic control of base-pair states, so acoustic energy couples to biology only classically, through pressure, strain, cavitation, and the gated release of molecular payloads. Against this boundary we audit the real deficiencies of in-vivo editing in solid tumours - penetration, endosomal escape, incomplete coverage, off-target activity, immune clearance of carriers, and clonal heterogeneity - and compare the traditional pathways (viral vectors, lipid nanoparticles, ex-vivo cell engineering, electroporation) with emerging ones (base and prime editors, sonoporation, tumour-derived extracellular vesicles and cancer-cell-membrane-coated particles, optical barcoding, alkaloid co-payloads). We then treat mutation mapping as the coordinate system of the problem, comparing single-cell multi-omics (GoT, TARGET-seq, GoT-ChA, TISCC-seq) with spatial platforms (Visium HD, Stereo-seq, Xenium, CosMx, MERFISH, GeoMx) and their integration with DNA data (Tumoroscope, CalicoST). Six structural gaps are identified and status-mapped: epigenetic and non-genetic resistance, immune clearance as the true effector, evolutionary dynamics under partial editing, safety and controllability architecture, integration with approved modalities, and the residue of the informational question. The resulting framework abandons the goal of editing every cell. It proposes instead a staged sequence - map, prime, transiently edit, clear immunologically, and strike again at the evolutionary nadir - in which controlled cavitation is the spatial actuator and antigen-release step, transient editors are the state-reset tool, and adaptive immunity plus evolutionary timing handle the residual population. Every component exists in experimental or early clinical form; none requires new physics. The missing work is their systematic combination, dose-timing, and longitudinal readout in immunocompetent models.
