An overview of the immunologic mechanisms and clinical features of irAEs is provided, discussing how the CTLA-4 and PD-1/PD-L1 immune checkpoints help maintain immune balance, and how their blockade results in unchecked immune activation.

Abstract

Immunotherapy has become a mainstay of cancer treatment by enhancing the immune system's ability to recognize and eliminate tumors. The use of immune checkpoint inhibitors (ICIs) has significantly improved survival across a wide range of malignancies but also introduced a distinct category of toxicities. The immune activation triggered by ICIs is responsible for distinct immune-related adverse events (irAEs) ranging from mild, self-limited inflammation to severe, life-threatening organ damage. Here we provide an overview of the immunologic mechanisms and clinical features of irAEs. We discuss how the CTLA-4 and PD-1/PD-L1 immune checkpoints help maintain immune balance, and how their blockade results in unchecked immune activation. To place these observations in context, we compare irAEs with monogenic disorders of immune dysregulation affecting the same pathways to illustrate the overlapping pathogenic mechanisms. We review the presentation and immunopathology of organ-specific irAEs, and examine current approaches to clinical management, highlighting the use of targeted treatment approaches to control toxicity while preserving anti-tumor efficacy.

Harnessing tumor immune checkpoints for autoimmune disease therapeutics

Current IC-targeted strategies hold promises but demand context-aware, mechanistically informed development for broad clinical impact in ADs, and future progress will require multi-omics profiling, biomarker-driven stratification, and next-generation agonist design.

Host determinants of immune-related adverse events during immune checkpoint inhibition

Immune checkpoint inhibitors (ICIs) have transformed the treatment of many cancers, but immune-related adverse events (irAEs) have become their principal limitation. This review argues that irAEs are not a random hazard of checkpoint blockade but a largely host-determined outcome layered on a drug-set baseline. The reg...

Immune checkpoint inhibitors and the inflammatory dysregulation: decoding cutaneous and gastrointestinal immune-related adverse events for the allergist and immunologist

Immune checkpoint inhibitors (ICIs) targeting programmed cell death protein 1 (PD-1), its ligand (PD-L1), and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) have revolutionized oncology, yet they unleash a unique spectrum of inflammatory toxicities known as immune-related adverse events (irAEs). Cutaneous and gas...
